No hard-copy forms are required. The digital forms may be emailed to each participant or parent/guardian based on the contact information provided, and registration is not fully complete until all required release forms are submitted.

A $50 late fee is per participant starting June 1, 2026 and an updated invoice will be sent unless other arrangements have been made with campyearbook@gmail.com and Rep Jason Castleton.

Cancellation Policy
Checks should be made payable to Camp Yearbook and mailed to Camp Yearbook, 4756 S Quail Point Road, SLC, UT 84124. If, for some reason, the payment cannot be made right away, please make sure all digital release forms are submitted before school is out and the students are on summer vacation. • Please send EVERYTHING in before Memorial Day Weekend if possible! • Cancellations made before May 31st will receive a full refund. • After May 31st, there will be a $100 cancellation fee per person. • No refunds will be given after June 6th. NO EXCEPTIONS • Accounts not paid in full by June 1st will be charged $50 per person unless arrangements have been made.
