I, the undersigned, am the above-noted winner. In consideration of being selected as a winner in the Contest and my receipt of the above-noted prize, I hereby declare, agree and confirm as follows:

1. I am a resident of the Province/Territory of Insert Province or Territory* and was 19 or older at the time I entered the Contest. Neither I nor any member of my immediate family or any person with whom I am domiciled is an employee of FoodMesh or its advertising and promotional agencies or the immediate family of such employees and persons with whom such employees are domiciled.

2. I accept the above-noted prize as awarded by FoodMesh.

3. I acknowledge and agree that I have read and fully complied with the Contest Terms & Conditions and that any breach of the Contest Terms & Conditions will result in the immediate forfeiture of the above-noted prize.

4. I hereby release and forever discharge FoodMesh, The Waste and Resources Action Programme (WRAP), and their respective affiliates, directors, officers, employees, and agents, and any other persons acting for or on behalf of FoodMesh in the Contest (collectively, the “Releasees”), of and from any and all actions, causes of action, claims, suits, demands, damages, interest, expenses or compensation of whatever kind, whether known or unknown, suspected or unsuspected, arising in law or in equity, including without limitation any matters arising in connection with participating in the Contest, receiving and making use of any prize awarded, and any errors or omissions in the offer or administration of this Contest, including but not limited to errors in the advertising, the Contest Terms & Conditions, the selection and announcement of the winners or the distribution of any prize.

5. I hereby agree to indemnify and hold harmless the Releasees from any and all liability for any injuries, loss or damage of any kind to me or any other person, including personal injury, death, or property damage, resulting in whole or in part, directly or indirectly, from acceptance, possession, use or misuse of any prize, participation in the Contest, any breach of the Contest Terms & Conditions, or in any prize-related activity.

6. The provisions of this release and acknowledgement shall ensure to the benefit of the respective heirs, executors, administrators, personal representatives, successors and assigns of each of the Releasees and shall be binding upon my heirs, executors, administrators and personal representatives.

7. This Release will be construed in accordance with and governed by the laws of the Province of British Columbia and the laws of Canada in force in the Province of British Columbia.
