Legal Disclaimer

• I understand that tattooing is the insertion of ink into the skin, designed to leave a permanent mark. I understand that a tattoo is a permanent change to my appearance and can only be removed by laser tattoo removal, which can be costly and may not result in complete removal.
  
  
• I am aware and informed of the inherent risks associated with getting a tattoo. These risks include but are not limited to: infection, scarring, difficulties in detecting melanoma, allergic reactions to tattoo pigment or latex gloves.
  
  
• I acknowledge it is not possible for the artist to determine whether I might have an allergic reaction to the pigments or processes used for my tattoo, and I agree to accept the risk that such a reaction is possible.
  
  
• I understand that tattoos can fade or lose clarity over time due the natural dispersion of pigment under the skin. I am aware that variations in the colour/design may exist between the design I have selected and the actual tattoo. I understand that because tattoos are created by hand and heal differently on each person, the appearance of the final tattoo may differ slightly from the original artwork, and change over time.
  
  
• I have checked the design for errors, and agree that neither the Artist or the Tattoo Studio is responsible for the meaning or spelling of the symbol or text that I have chosen to get tattooed.
  
  
• I will follow the verbal and written aftercare instructions which have been given to me. I acknowledge that infection is always possible as a result of being tattooed, particularly in the event that I do not follow aftercare instructions, and that any such infection is my responsibility, and not the responsibility of the Artist. If any touch-up work to the tattoo is needed due to my own negligence, I agree that the work will be done at my own expense.
  
  
• I agree to release, discharge and hold harmless the Artist and Tattoo Studio all liabiity from any and all claims, damages and legal actions arising from or connected in any way with my tattoo and the procedures used to apply my tattoo, including direct or consequential damages which arise from my tattoo, whether the fault of either the Artist or the Tattoo Studio or otherwise. I agree to reimburse the Artist for any legal fees and costs incurred in any legal action I bring against either the Artist or the Tattoo Studio. I understand that by signing this document I am signing a legal contract.
  
  
• I consent to this data being held on file for a period of two years, as required by licensing regulations. 

Medical Disclaimer

• To my knowledge, I do not have a physical, mental or medical impairment or disability which might affect my wellbeing as a result of my decision to have a tattoo.
  
  
• I am not pregnant or breastfeeding.
  
  
• I understand that every care will be taken to ensure that the procedure is carried out in a hygienic way, which includes the use of single use and pre-sterilised equipment.
  
  
• I understand that disclosure of HIV or Hepatitis status is unnecessary and invasive.  nowing a client has a blood borne virus does not change the application or processes of the tattoo, as utmost safety precautions are undertaken with every client. HIV and Hepatitis are not contraindications to tattooing, piercing or cosmetic procedures. The British HIV Association of specialist doctors states: “Collecting information about HIV status must be justifiable, as per current data protection legislation (Data Protection Act 2018) and is unnecessary in the context of tattooing, piercing and cosmetic or routine beauty treatments”. 
  
  
• I understand that the tattoo healing process places extra load on my immune system, and I judge my current health to be in good enough standing that I am not taking on a significant or unnecessary risk to my wellbeing by getting a tattoo.
  
  
• I have undertaken my own research to ensure that any medications I am on will not interfere with the application or healing of the tattoo.
  
  
• I have expressly notified the Artist of any health contraindications that may affect the application or healing of the tattoo. Contraindications include:
  - Blood thinning agents
  - High/Low Blood pressure 
  - Hemophilia/other clotting disorders or a history of haemorrhaging
  - Diabetes 
  - Epilepsy 
  - Allergies e.g. adhesives / metals / latex / seed oils 
  - Cancer 
  - Heart disease
  - Pace-Maker 
  - Fainting attacks / POTS 
  - Anaemia 
  - Auto-immune disease 
  - Moles, acne, eczema, psoriasis or sunburn in the area to be tattooed
