LIVE EVERY DREAM TRAINING – SUMMER CAMP WAIVER

I, the undersigned parent or legal guardian, give permission for my child to participate in Live Every Dream Training soccer camps, training sessions, drills, games, and related activities.

I understand that participation in physical and sports activities carries inherent risks, including but not limited to injury, illness, dehydration, falls, or other physical harm. I voluntarily assume all risks associated with my child’s participation.

I hereby release and hold harmless Live Every Dream Training, its owners, coaches, staff, volunteers, sponsors, and affiliated facilities from any liability, claims, demands, or causes of action arising from injury, illness, or damages related to participation.

I authorize Live Every Dream Training staff to seek emergency medical treatment for my child if necessary and understand that I am financially responsible for any medical expenses incurred.

I also grant permission for photos/videos of my child to be used for promotional, marketing, and social media purposes unless otherwise stated in writing.

I understand that registration fees may be non-refundable and that all participants are expected to follow camp rules and demonstrate respectful behavior.

Camp Levels & Prices & times :

Beginners Group (2 Hours Daily) – $85 (for the 3 days)

Advanced Group (2 Hours Daily) – $100 (for the 3 days)

JR / Babies Group (1 Hour Daily) – $60 (for the 3 days)
