  • DAILY SCAFFOLD INSPECTION CHECKLIST

  • This form must be filled out by a Competent Person before each shift and after any occurrence that could affect the structural integrity of the scaffold. 

    Each sub is responsible to complete their own, individual daily use inspection. 

     

  • Ladder or other suitable means of access is in place*
  • Scaffold is protected from construction traffic*
  • Scaffold is plumb and level*
  • All footings are level, rigid, and capable of supporting the load*
  • Sills are proper size, in place, and centered under base plates*
  • Legs are braced and braces are properly attached*
  • Toe boards are in place*
  • Screw-jacks have firm contact with frame and base plates pinned to frame*
  • Height to base ratio exceeds 3 to1 and scaffold is restrained from tipping*
  • Platforms are within 14" of the finished face of the building*
  • All working levels are fully planked, at least 18" wide, and not used if slippery*
  • Planks overlap at least 12" and overlap is over a frame bearer*
  • Plank overhang is 6-12" on 10' planks (6-18" on planks longer than 10')*
  • Cleats are installed on the end of planks when planks don't overhang at least 6"
  • Planks don't deflect more than 2" in the middle and are in good condition*
  • Gaps between planks are not greater than 1"*
  • Guardrails are in place on sides and ends of platforms or workers protected by a PFAS*
  • Workers below are protected with screens, signs, or barricades; and workers are utilizing tool tethers when people may work or walk below.*
  • Scaffolds, materials, and workers are at least 10' away from power lines*
  • Other companies using scaffold have signed the BEK equipment loan & Indemnification*
