  • iHV Health Visitor/ Health Visiting Team of the Year Award 2026 - Submission Form

    There are excellent health visitors and health visitor teams working across the NHS, social care and independent sector – and we want to celebrate those who are truly exceptional. The categories recognise either an individual or team working in the NHS, social care, charitable or independent sector who have made an outstanding contribution to our profession - driving best practice, transforming care, improving outcomes and demonstrating excellence in health visiting.

  • The submission deadline for entries is 5pm on 6 July 2026.  We welcome entries from candidates themselves or nominations from those who wish to highlight an individual health visitor or health visiting team who they believe merit this high level of recognition. Submissions will be accepted from individuals and teams with current iHV membership.  Entrants must have UK or British Crown Dependency status.

  • Health Visitor of the year category:

    Candidates can be at any stage in their health visiting career, but should have undertaken a high-quality project or service improvement on their own initiative which has required innovation, determination and creativity, whilst working alongside others to spread excellence. Candidates must demonstrate how they have driven excellence in health visiting, using evidence to support their submission- including: why the project was needed, how they have engaged with target populations, application of best evidence, what it involves, and what success looks like- supported by indicators, outputs and outcome measures to demonstrate the tangible positive impact their work has had.

    Health Visiting team of the year category:

    Health visiting teams can be new or well established. Regardless of maturity they must have undertaken a piece of work, met a challenge or delivered a service improvement that has required collaboration, determination and creativity. They must have drawn on the talents of individuals and cooperated collectively. Team cooperation and collaboration must demonstrate how they have driven excellence in health visiting., Using evidence to support their submissions- including: why the project was needed, how they have engaged with target populations, application of best evidence, what it involves; and what success looks like- supported by indicators, outputs and outcome measures to demonstrate the tangible positive impact their work has had.

     

  • The Practice Award submission must include:

    1. A fully completed application form – remember your application needs to stand alone as this will be the primary material being judged.

    2. A short 60 second video presentation by the candidate/team or nominee explaining what has been achieved and why the submission should be considered for an iHV award. This will provide the judges with an introduction to the candidate or team who has excelled in practice.

    3. Up to 3 additional pieces of supporting evidence - for example publications, testimonies, a data summary table or image etc. Supporting evidence will be used by judges if there is a need to differentiate merit between submissions. The supporting evidence must be concise and relevant to specific claims within the application. Only a maximum of three pieces (as single pages) of evidence will be considered, so do not submit any more.

    Should the nominee be successfully shortlisted, their attendance will be required at an award ceremony in London on Tuesday 6th October 2026, 5-7pm
  • GDPR Statement

    This event is sponsored by the iHV. The names and workplaces of those shortlisted and winning the awards will be displayed on the iHV website. Statements and videos submitted by applicants may be used as part of celebrating the success of the achievements of health visitors by inclusion within an iHV website or blog. Submitted video footage may also be used by the iHV as part of ongoing promotion, in print, or at the awards ceremony.
