We at Southeastern Mississippi Youth Camp are honored that you feel led to serve as a counselor this year. Due to the times in which we live, it is required that all counselors complete this form in full.
We also ask that your pastor sign and date this form at the bottom to verify, to the best of their knowledge, that the information provided is accurate and that you are eligible to serve as a counselor under our guidelines.
Counselor Requirements:
• You must be faithfully committed to a home church with at least six (6) months of consistent attendance.
• All individuals must be free from harmful habits, including but not limited to tobacco, alcohol, drugs, vaping, or similar substances. These are strictly prohibited on camp premises, and any violation will result in immediate removal without return.
• You must live a morally upright Christian life, free from lifestyles or behaviors such as adultery, fornication, homosexuality, pornography, or other conduct that would hinder your role as a counselor.
• Individuals who have been convicted of any sexually related crimes are not eligible to serve as a counselor.
• You must be willing to participate in all assigned activities and serve both the Lord and the campers throughout the week.
• You must abide by all camp rules. Any counselor may be dismissed immediately if their behavior is misleading or not reflective of Christian character.

By signing this form, you give Southeastern Mississippi Youth Camp Outreach Ministries, Inc. permission to conduct a background check.

• If any laws are violated during camp, local law enforcement will be contacted.
