Informed Consent and Acknowledgement
1. Acknowledgment of Risks: I acknowledge that participation in soccer training and activities offered by SoccerStars Academy involves inherent risks of physical injury. These risks may arise from actions, negligence, or omissions of participants, coaches, staff, or third parties, the condition of the training facilities, equipment, or weather conditions.
2. Assumption of Responsibility: I understand and accept full responsibility for my own safety and well-being during my participation in the SoccerStars Academy. I am aware of the risks involved, and I voluntarily choose to participate, recognizing that my participation may involve physical exertion and potential contact with other participants.
3. Medical Information: I affirm that I am in good physical condition and have no known medical conditions that would prevent my participation in the soccer academy. I agree to inform the academy staff and coaches of any changes to my health or medical condition that may affect my ability to participate safely.
4. Waiver of Liability: In consideration of being allowed to participate in the soccer academy, I hereby release, waive, and discharge SoccerStars Academy, its officers, directors, employees, coaches, volunteers, agents, and sponsors (the “Released Parties”) from any and all liabilities, claims, demands, or causes of action whatsoever arising out of my participation in the soccer academy, including those caused by the negligent acts or omissions of any of the Released Parties.
5. Indemnification: I agree to indemnify and hold harmless the Released Parties from any claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney's fees, arising out of my participation in the soccer academy.
6. Use of Likeness: I grant permission to SoccerStars Academy to use my name, image, or likeness in promotional materials, photographs, or videos related to the soccer academy without compensation to create media for educational and promotional purposes.
By signing this Waiver and Liability Agreement, I acknowledge that I HAVE READ AND FULLY UNDERSTAND AND AGREE TO ALL OF ITS TERMS AND CONDITIONS INCLUDING PERMISSION TO TREAT AGREEMENT. I further state that I have executed this waiver and liability voluntarily and with full knowledge of its significance to be binding on my, my heirs, executors, administrators and assigns.
I agree to the above statement
