  • This Hold Harmless Agreement is between the parent or legal guardian of the participant and Jackson Youth Volleyball.

    1. Acknowledgment of Risk

    The undersigned, as the parent or legal guardian of the Participant, acknowledges that participation in volleyball activities, including but not limited to practices, games, and other related events, involves inherent risks, including but not limited to physical injury. The undersigned understands and acknowledges that such risks cannot be completely eliminated.

    2. Assumption of Risk

    The undersigned voluntarily assumes all risks and hazards associated with the Participant’s involvement in the activities organized by Jackson Youth Volleyball. This includes, but is not limited to, personal injury, property damage, or other losses that may arise out of or relate to participation in these activities.

    3. Hold Harmless and Indemnification

    The undersigned agrees to hold harmless, indemnify, and defend Jackson Youth Volleyball, its officers, directors, employees, volunteers, agents, and affiliates (collectively, “Releasees”) from and against any and all claims, demands, causes of action, suits, judgments, and liabilities (including legal fees and expenses) arising out of or related to any injury, loss, or damage suffered by the Participant during or in connection with their participation in the activities organized by Jackson Youth Volleyball.

    4. Medical Treatment

    In the event of an emergency, the undersigned authorizes Jackson Youth Volleyball or its representatives to seek medical treatment for the Participant. The undersigned agrees to be responsible for any medical expenses incurred as a result of such treatment.

    5. Entire Agreement

    This Agreement constitutes the entire understanding between the undersigned and Jackson Youth Volleyball regarding the subject matter hereof and supersedes all prior agreements, understandings, or representations. This Agreement may not be altered or amended except in writing signed by both parties.

    6. Governing Law

    This Agreement shall be governed by and construed in accordance with the laws of the State of Ohio, without regard to its conflicts of law principles.

    7. Severability

    If any provision of this Agreement is found to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.

    8. Acknowledgment of Understanding

    The undersigned acknowledges that they have read and understood the terms of this Agreement and that they are signing it voluntarily.

    By signing below, I hereby agree that the Jackson Youth Volleyball Program shall not be liable for any injury or loss which may be sustained while participating in the Jackson Youth Volleyball program.


    I, as a parent or legal guardian, will be responsible for all medical charges relating to my child during this time. The above applicant is in good health and has my permission to participate in the program

    Unfortunately, due to rising incidents of violence at youth sporting events around the nation and some in Ohio, including our area, this league feels obligated to institute a parental/guardian/custodian code of conduct.  In order for your child to participate in our volleyball league, you must sign and adhere to the conditions of this agreement.  The terms and conditions of the parental code of conduct are as follows:


    Parents are encouraged to root for their team in a positive manner.  Parental
    involvement is the most important factor in a child’s development.  Please let this be a positive experience.  Remember, it is only a game.  All of these children will be attending the same school in the 6th grade.


    Parents agree NOT to do any of the following:

    1. Curse at anyone whether it be a player, fan, coach, referee, etc. before, during or after a game or practice.
    2. Throw any objects onto the floor during a game or practice.
    3. Come onto the floor for any reason unless your child is injured and then only when directed to by a coach or official.
    4. Strike or threaten to strike or act in any sort of threatening manner toward a player, coach, spectator or official.
    5. Use any sort of alcohol or tobacco in the gymnasium.
    6. Show up in an intoxicated state at the games or practices.
    7. Act in any other unsportsmanlike manner.

    This list is not meant to be all-inclusive, but to serve as examples of unacceptable behavior.

    Penalties for violations of the Code of Conduct:

    For lesser infractions, you will be warned once by a game official or league officer.  If further violations occur, you and your child, whether he/she is in any way at fault or not, will be ejected from the game and gymnasium and will not be allowed to participate in the league for the rest of the year.

    For more serious violations including but not limited to the above, in addition to being ejected along with your child from the gymnasium immediately, your case could be referred to local law enforcement officials for legal action.

    I fully support and understand the Parental Code of Conduct and will encourage my child(ren) in the league to act in a sportsmanlike manner.


    I also fully understand the consequences to my child(ren) and me for my failure to adhere to the Parental Code of Conduct
