In submitting this form, artists are declaring that:

1. To the best of their knowledge:

1.1they know of no reason from their past or present personal, professional or financial affairs, including their use of social or other online media, why their appearance would or could bring public disgrace, conflict or ridicule to the product/agency/advertiser;

1.2 they are able to undertake this work on the proposed production dates; and

1.3 therefore, there are no particular issues or potential reasons why they may not be able to undertake the work on the scheduled date(s

2. They agree to keep fully confidential all information received relating to the casting, including details of advertisers, the advertiser’s product, the intended commercial and any shoot or air dates.

3. They are also agreeing that if successfully cast for this role, they will produce a medical certificate if it should prove necessary, advising on their fitness or otherwise to work on this production.

Declaration

The information you provide on this form will only be used for the purposes of this casting process. If you are unsuccessful, it will only be retained for as long as we deem reasonably necessary for our record keeping purposes. If you are successful, it may be kept for as long as the ad is used. It may be shared with those involved in the ad’s production, such as the client/advertiser, agency, casting agent, your agent, production company and the insurers of the ad. In certain cases, you may request access to and the correction or erasing of your information or the restriction of its processing.

Your information will only be processed where there is a legal basis for doing so. Generally, it will be used to help decide whether to enter into a contract with you to perform in the ad, or for legitimate interests such as evaluating your performance at the casting, or based on your consent (for example with regard to any sensitive information such as your racial or ethnic origin, or your health).
