Accessible Visit - First Visit
Please note, this form is for employed carers or support workers to request a visit with free carer tickets; please DO NOT fill it out if you are an in-family carer (e.g. spouse, parent, sibling or child) with a disabled child or adult in your family. This is a form for a FIRST VISIT to the farm. If you have been before but not yet filled out this form, in person or online, then please treat it as FIRST VISIT. Then, for subsequent visits there is a much simpler form to indicate the intended date and to trigger a repeat issue of the free carer tickets.
Why do carers and support workers need to fill in this form?
This form helps us understand any support requirements before your visit so that we can make reasonable adjustments where practicable and ensure a safe, enjoyable experience for everyone. It also enables you to request free entry for carers. If you are a carer visiting on different occasions with different clients, then you will need to fill out this form for each client. We will keep a paper copy of the form in a folder in a secure location as reference for future visits so you will only need to fill it out for the first visit for each specific client you will be attending with. Periodically we may ask you to update or confirm the information is still accurate and we would also ask you to volunteer any information that has changed such as changes in needs or behaviours that would help us ensure a safe visit. THANK YOU FOR YOUR COOPERATION!
Is this a booking?
This is not your booking. It is a registration of your client and yourself so that we can validate / authorise carer entry when you arrive. On arrival, please state that you have filled in a pre-registration form, or ask to fill one out when you arrive if you prefer not to submit this form online. After submission, we will assess your information and contact you to confirm your booking and free carer(s) ticket.

Play Barn
Entry Policy Play Barn
The Big Play Barn is designed for children aged 12 years and under and is not available for general use by older visitors. The following rules apply equally to all visitors: Children aged 12 years and under are welcome to use the play equipment. Visitors aged 13 years and over may enter the Play Barn only when accompanying and supervising a younger sibling or close family member aged 12 years or under. Admission to the Play Barn will NOT be permitted for teenagers (13+) or adults in any other circumstances. These age restrictions are based on the design, safe operation and intended use of the play equipment and apply to all visitors.
Entry Policy Farm
The Farm Park welcomes visitors of all ages, and teenagers and adults are welcome to enjoy the animals and the wider attraction. Many of our organised animal activities, such as handling sessions and interactive experiences, are specifically designed for young children. To ensure the welfare of our animals and provide the best experience for our younger visitors, these activities will generally be reserved for children. Participation by older visitors may be permitted at the discretion of our staff where it is appropriate, safe and practical to do so.
Describe any support or reasonable adjustments that would help make your visit safe, comfortable and enjoyable. We will do our best to accommodate your requirements wherever reasonably practicable and will contact you before your visit if we need to discuss any arrangements.
Describe any mobility or physical access requirements that may affect your visit. The site is largely accessible, but some areas and activities may be affected by terrain, weather conditions or the nature of the attraction. Providing this information in advance helps us make reasonable adjustments where practicable and ensure you have the best possible experience.
Describe any behavioural, communication or sensory needs, known triggers or other support requirements that may help us plan your visit safely. If the visitor may become distressed or display behaviours that could affect either their own safety, the safety of others or animal welfare, please describe the circumstances, any known triggers, and the strategies or support plans that are effective in preventing or de-escalating the situation.
I confirm that the supervising carer will remain responsible for the supervision and support of the visitor throughout the visit and will provide the level of supervision reasonably required to ensure their safety and the safety and wellbeing of other visitors, staff and animals.
