Nipple Prosthetic Order Form

These nipple prosthetics are carefully handcrafted to order by Elaine Abich, founder of Evolve Med Ink. As a paramedical tattoo artist working closely with individuals after mastectomy and breast reconstruction, she wanted to offer and additional option for those who may not want 3D areola & nipple tattoos, or who are planning to move forward with it but are still healing from their reconstruction surgery and not quite ready for their tattoos yet.

Each prosthetic is made from high-quality, medical grade silicone that is safe for sensitive skin. They are designed to look incredibly realistic, with soft dimension and natural tones, and feature ultra-thin edges that allow them to blend smoothly into the surrounding skin for a natural, undetactable finish.

Because each piece is handcrafted, slight variations in color and design are expected - even within a pair - which helps create an even more natural appearance. No two areolas and nipples are exactly the same, so every set is uniquely made for you.

Each order will include your prosthetic (s) and a small starter supply of medical adhesive, typically enough for approximately 2-3 applications.

INTRODUCTORY PRICING (now through Jun 15, 2026)

I am currently offering introductory pricing as I expand my shade collection and put the finishing touches on my full packaging experience. This is a great time to try Bare at a lower price — quality and craftsmanship remain the same.

• Single prosthetic (one areola/nipple): $100
• Pair (two prosthetics): $175
• No sales tax will be applied during this introductory period

Please note: Introductory pricing applies to ready-made prosthetics only — preselected colors and designs from our current available inventory. Custom orders are not subject to introductory pricing

 

REGULAR PRICING (starting June 16, 2026)

• Single prosthetic: $150
• Pair: $275

FITTING APPOINTMENT (Optional — Riverside, CA only)

• In-person fitting: $50 (standalone service, by appointment only)
• 30-minute private appointment with artist Elaine Abich

 

CUSTOM ORDERS 

More than a color match — we are recreating your biological areolas and nipples as closely as possible to your natural ones. Using a mold impression of your existing or pre-surgery areola and nipple, each prosthetic is handcrafted to replicate your unique shape, texture, and color — so that what you wear feels truly yours.

Please note: Because each piece is handcrafted by hand, results will be as close as possible to your natural areola and nipple but may not be an exact replica. No two handcrafted pieces are identical — this is part of what makes each one unique.

- Unilateral (one side/ one prosthetic): $325 — includes mold impression, color matching, one custom prosthetic, fitting appointment, application/removal walkthrough, and one bottle of adhesive

- Bilateral (one pair): $650 — includes everything above for both sides plus one bottle of adhesive

Because your custom order is color-matched directly to your skin by the artist in person, you receive one matched pair — not multiple variations. Custom orders are available in Riverside, CA only and are by appointment only. Custom orders are not subject to introductory pricing.

PAYMENT

Payment is accepted via Zelle or Venmo. You will receive payment details after your order is finalized.

A fitting appointment is a private 30-minute session where you get to experience Bare prosthetics in person before committing to an order. Here is what to expect:

- See the full shade range in person and try different colors directly against your skin for a true match
- Try on prosthetics to find your perfect size and feel
- Select your preferred shade and size with confidence
- If your chosen shade and size is available in studio that day, you are welcome to purchase and take it home with you — no waiting, no shipping
- We will walk you through how to properly apply and remove your prosthetic so you feel completely comfortable from day one

Fitting appointments are $50 and take place in Riverside, CA only, on selected Wednesdays, Saturdays, and Sundays. All appointments are by appointment only and must be booked in advance. The $50 fee is a standalone service and is not applied toward your prosthetic order.

This service is designed for patients preparing for an upcoming mastectomy who would like to preserve a professional mold of their natural areola and nipple before surgery — just in case they decide to move forward with prosthetics at any point in their healing journey.

By submitting this form, you understand this is a two-step process. Step 1 is this form — sharing your preferences. Step 2 is your final selection — I will follow up via email within two business days with photos of available options in your size and color range. No payment is collected until you have reviewed your options and confirmed your final selection. You also confirm that you have reviewed the product details, pricing, warranty, and shipping information.
