    PARENT PARTICIPATION NOTE
    Our monthly Family Formation nights are designed for both parents and children in grades K–5. While children participate in age-appropriate classroom formation, parents are invited to attend a separate formation session created especially for them.

    DOES YOUR CHILD WISH TO PREPARE FOR A SACRAMENT?
    If your child was not baptized at St. Luke, you will be asked to provide a copy of their baptismal certificate as part of this registration.

    REGISTERING A STUDENT IN GRADES 9–12?
    If you have a student in grades 9–12 who is interested in growing in their faith, please contact Kristen Zacek at k.zacek@stlukegvsu.org for more information.

  • Registering your teen for 8th grade faith formation does not automatically enroll them in Confirmation preparation. Each teen who desires confirmation must attend the information meeting along with at least one parent. The Info meeting will be held Sunday, September 20th from 6:30-7:30pm. 

  • First Communion Entrance Interview

  • Please reach out to Kristen Zacek (k.zacek@stlukegvsu.org) to sign up for an entrance interview (please note: at least one parent is expected to be present during the interview). Your child WILL NOT be enrolled in First Communion prep until after they meet with Kristen.

  • Medical Treatment Release

  • As a parent/guardian, I do hereby authorize the treatment by a qualified and licensed physician of any condition, which, in the opinion of the physician, is deemed necessary and appropriate. This authority is granted only after a reasonable effort has been made to reach me.

    I further authorize the person who presents the minor to sign the Acknowledgement of Receipt of Notice Privacy Rights that may be presented by the physician or health care facility.

    This authorization is completed and signed of my own free will with the sole purpose of authorizing medical treatment deemed necessary and appropriate by the treating physician.

    This ONE-TIME registration fee covers: books/resources, adult/parent formation, and child formation for the YEAR!

    • Families with 1 Child = $150
    • Families with 2+ Children = $250

    We believe participation is an important part of a student's faith formation, so no family will be denied due to financial hardship. Please contact Kristen Zacek at k.zacek@stlukegvsu.org to confidentially discuss your needs.
