Siblings are welcome, but must enroll in separate sessions (one sibling per session). Please fill out the form twice.

Our activities are designed for older youth, and younger children may find the projects and tools challenging or frustrating. To help ensure a safe, positive experience for everyone, please register using your child’s correct age.

WAIVER FOR PARTICIPATION

Parent/Guardian Acceptance of Responsibility

I am aware that while participating in an Española Valley Fiber Arts Center program or activity, certain risks and dangers may be present, including but not limited to working in a studio environment with special tools like scissors, gas powered stoves and boiling water. I concur that the participant must comply with all rules of the program and directives of EVFAC's staff. To ensure participant's successful participation in the program, I will fully disclose any medical conditions and any need for special accommodations of which EVFAC staff should be aware.

The participant will be appropriately dressed for the daily activities: casual clothes that are ok to get messy, comfortable shoes, and sun protection for being outside. Should emergency conditions require medical care for the participant, EVFAC has my consent to secure treatment at the closest hospital and I am responsible for full payment of the resultant costs.

I hereby release and discharge the Española Valley Fiber Arts Center, its officers, employees, agents, teachers, and volunteers from all liability arising out of or in connection with the above described program of activities, including, for the purpose of this agreement, all claims that I, my heirs, or assignees may have against EVFAC because of any personal injury, illness, or because of any loss or damage to property that occurs during the above program.

PHOTO RELEASE WAIVER

I hereby grant permission to the rights of my child's image, likeness and sound of voice as recorded on video or audio. I understand that my child's image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my child's likeness appears.

By agreeing to this release I understand this permission signifies that photographic or video recordings of my child may be used for any promotional or educational purposes including posters, brochures, social media, website, blog, radio broadcast, television and other promotional outputs.

There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed.
