Agreements, Consent & Participant Acknowledgment
Please read and confirm the following:
I understand that my participation in Front Porch Broadway: ROCK THE BLOCK FEST is voluntary and unpaid unless otherwise agreed in writing. I agree to participate respectfully and to honor the community, stories, and individuals involved in this project.
I understand that my participation may include rehearsals, performances, movement, walking between locations, studio recording, and participation in indoor and outdoor community spaces. I voluntarily assume any risks associated with these activities and accept responsibility for my personal safety and belongings.
I give permission for my name, image, likeness, voice, performance, interview, and participation to be photographed, recorded, and used for project-related purposes, including documentation, promotion, social media, website and app content, press, and archival use.
I understand that any stories, ideas, words, music, or creative contributions I share may be adapted, combined, performed, recorded, and used as part of songs, scripts, performances, digital content, and archival materials connected to the project.
I understand that final creative works developed through this project may involve multiple contributors and may be owned, managed, and used by the project organizers and partners for ongoing project, promotional, educational, and archival purposes. I understand that participation does not guarantee compensation, ownership, credit, or approval rights unless outlined in a separate written agreement.
To the fullest extent permitted by law, I release and hold harmless the project organizers, artists, partners, sponsors, and affiliated organizations from liability related to my participation, except in cases of gross negligence or intentional misconduct.
