If applying for a Matching Scholarship, please include below the name and address of the organization and the contact person who will be writing a letter pledging funds on your behalf.
If I am applying for a Matching Scholarship, my sponsoring organization will send the letter of recommendation to president@madog.org by Monday, June 1st 2026, confirming the amount of money pledged on my behalf.
