ONE LA SALLE FOOTBALL FESTIVAL 2026

PARTICIPANT WAIVER, RELEASE OF LIABILITY, MEDICAL AUTHORIZATION, MEDIA CONSENT, AND DATA PRIVACY AGREEMENT

By registering for and participating in the One La Salle Football Festival 2026 (“Tournament”), I, as participant or parent/legal guardian of the participant (if below 18 years old), acknowledge, understand, and voluntarily agree to the following terms and conditions:

1. ASSUMPTION OF RISK
I acknowledge that participation in football and related tournament activities involves inherent risks, including but not limited to:

• Physical contact and collisions with other players
• Falls, slips, and accidents
• Sprains, fractures, muscle injuries, concussion, or other bodily injuries
• Heat exhaustion, dehydration, or weather-related medical conditions
• Risks arising from field conditions, equipment use, and transportation to/from the venue

I voluntarily assume all risks associated with participation in the Tournament.

2. RELEASE AND WAIVER OF LIABILITY
I hereby release, waive, discharge, and hold harmless the following from any and all claims, liabilities, demands, damages, actions, causes of action, costs, and expenses arising from participation in the Tournament:

• University of St. La Salle
• USLS Alumni Association
• One La Salle Football Festival 2026 Organizing Committee
• Event directors and coordinators
• Tournament officials and referees
• Coaches and team representatives
• Volunteers and staff
• Sponsors and partners
• Venue owners/operators
• Their respective officers, employees, and representatives     

This waiver applies to any injury, loss, damage, illness, accident, or expense arising directly or indirectly from participation, except in cases of proven gross negligence or willful misconduct.

3. MEDICAL FITNESS CERTIFICATION
I certify that:

• The participant is physically fit and medically capable of participating in competitive football activities;
• The participant has no medical condition that would make participation unsafe;
• Any existing medical condition relevant to participation has been disclosed to tournament organizers.

I accept full responsibility for the participant’s physical condition and readiness.

4. EMERGENCY MEDICAL AUTHORIZATION
In the event of injury, illness, or medical emergency during the Tournament, I authorize tournament officials to secure emergency medical treatment for the participant as deemed necessary.

I understand and agree that:

• Tournament organizers may act in the participant’s best interest when immediate medical attention is required;
• All medical, hospital, ambulance, and related expenses shall be solely my responsibility.

 

5. PARENT / GUARDIAN CONSENT (FOR MINORS)
If the participant is under eighteen (18) years of age, I certify that I am the participant’s parent or legal guardian and I:

• Consent to the participant’s registration and participation;
• Agree to all terms contained in this waiver on behalf of the participant;
• Accept full legal and financial responsibility for the participant.

6. TOURNAMENT RULES AND CODE OF CONDUCT
I agree that the participant shall comply with all:

• Tournament rules and regulations
• Referee decisions
• Venue policies
• Safety instructions
• Sportsmanship and conduct standards

I understand that tournament organizers reserve the right to suspend, disqualify, or remove any participant, coach, team official, or spectator for:

• Unsportsmanlike conduct
• Violence or abusive behavior
• Misrepresentation of eligibility
• Violations of tournament policies

Such action may be taken without refund.

7. EVENT MODIFICATION / CANCELLATION / NO REFUND POLICY
I acknowledge that tournament organizers reserve the right to:

• Adjust schedules
• Modify formats
• Reassign venues
• Postpone or cancel matches or the Tournament

due to:

• Weather conditions
• Safety concerns
• Venue issues
• Government restrictions
• Force majeure or circumstances beyond organizer control

Registration fees are non-refundable, except at the sole discretion of the organizers.

8. MEDIA RELEASE AND CONSENT
I grant permission to the Tournament organizers, University of St. La Salle, and affiliated partners to:

• Photograph
• Video record
• Live stream
• Publish images, videos, and likenesses of the participant.

Photos collected for this registration will be used solely for proper identification of registered players by officials before and during the games.

for use in:

• Event documentation
• Promotional materials
• Social media
• News releases
• Future tournament marketing

I understand that no compensation shall be provided for such use.

9. DATA PRIVACY CONSENT
In compliance with the Data Privacy Act of 2012, I consent to the collection, storage, processing, and use of personal information submitted through this registration form solely for:

• Tournament registration and administration
• Player verification
• Scheduling and event communication
• Safety and emergency coordination
• Official tournament documentation

Tournament organizers shall take reasonable steps to protect participant information and shall not disclose personal data to unauthorized third parties except when required by law.

10. ACCURACY OF INFORMATION
I certify that all information submitted in this registration form is complete, accurate, and truthful.

I understand that false, misleading, or incomplete information may result in disqualification.

 

11. ACKNOWLEDGMENT AND ELECTRONIC CONSENT
By checking the consent box and submitting this registration form, I acknowledge that:

☑ I understand that this electronic acceptance carries the same legal effect as a handwritten signature.
