Parental Consent and Liability Waiver
In consideration of participation in the 2026 Cattle Punchers Hockey Camp, I acknowledge and agree to the following:

Cattle Punchers Hockey reserves the right to adjust schedules, groupings, or player placement based on enrollment, skill level, and overall camp operations.

I understand that participation in hockey activities involves inherent risks, including the potential for injury. By registering, I voluntarily assume all risks associated with participation.

I hereby release and hold harmless Cattle Punchers Hockey, Signal Peak Energy Arena, and all coaches, staff, and affiliated personnel from any and all liability, claims, demands, or causes of action arising from injury, illness, loss, or damage that may occur during participation in the camp.

I certify that the participant is in good physical health and able to safely participate in hockey-related activities. I understand that I am responsible for any medical, dental, or insurance expenses that may arise from participation.

Cancellation Policy
All registrations are final. No cancellations or refunds will be issued.

Registration Acknowledgment
Submission of this form and completion of full payment are required to secure a spot in the camp. Registration is not guaranteed until payment has been received in full.

Additional Consent & Acknowledgment
A consent form must be completed for each camp or clinic registration. Submission of this form applies only to the specific session selected. Additional or third-party waivers may be required and will be provided if applicable.

Participants and families are expected to follow all facility rules and guidelines set by Cattle Punchers Hockey and Signal Peak Energy Arena during camp activities.

By registering, I acknowledge that participation in group athletic activities may involve exposure to illness or other health risks. I voluntarily accept these risks and agree to participate accordingly.
