Assumption of Risk and Release of Liability
I have been advised and am informed that there are inherent hazards in practicing aikido, jiu jitsu, and self-defense. Before enrolling, I have had the opportunity to observe classes and ask questions about the risks inherent in practicing aikido, jiu jitsu, and self-defense.
I understand that aikido, jiu jitsu, and self-defense involve physical contact, falling, stretching, tumbling, and strenuous body movement. I understand that occasional injuries occur and are a risk in taking part. I hereby personally assume all risk in connection with my aikido practice, jiu jitsu practice, and/or self-defense classes for any harm, injury, or damage that may befall me as a result of my participation, whether it be foreseen or unforeseen; and further agree to save and hold harmless Tenzan Aikido and all other persons associated therewith from any claim by me, or my family, estate, heirs, or assigns, arising out of my enrollment and participation in aikido, jiu jitsu, and/or self-defense practice.
I further state that I am of lawful age and legally competent to sign this affirmation and release, or that I have acquired the written consent of my parents or guardians; that I understand the terms herein are contractual and not a mere recital; and that I have signed this document of my own free act. It is my intention by this instrument to exempt and release Tenzan Aikido and all persons associated therewith from all liability whatsoever for personal injury, property damage, or wrongful death caused by negligence.
Adult Tuition Policy
Dues: Unlimited practice $175 monthly. Recurring Auto-Pay $170 monthly. Dues are paid on a monthly basis. Members must pay for the entire month in advance. Tuition is due within the first week of the month regardless of attendance.
Late Fee: $15. If tuition is not received within the first week of the month (by the 7th day), an additional $15 late fee will be charged with your monthly dues.

Registration Fee: $69. Family discounts available.
Leave of Absence / Cancellation of Auto-Pay: We require fourteen days' written notice for suspension or cancellation of dues. Forms are available at the dojo front desk.
Refunds: All fees, once paid, are non-refundable. Please consider carefully the program you are signing up for, because we cannot provide refunds.
Discounts: If you join with siblings or parents, there is a 10% family discount off the highest dues amount (may not be combined with other discounts).
Media Permission
I grant permission to Tenzan Aikido dojo to take and use visual/audio images of me and/or my children. I waive any right to inspect or approve any finished images that may be used online or in any media or printed material.
I have fully informed myself of the contents of this affirmation and release by reading it before I signed it. I have read, understand, and agree to follow the above-stated Tenzan Dojo policies.
