This is form for my flash tattoo designs. I am located at Calico Ink Tattoo in Acworth, Ga! If you would like a flash design, please fill out the form below. To book an appointment I do require a $100 non-refundable deposit to secure your appointment date and it goes towards the total cost of the tattoo. Thank you ❤

Are you 18 years of age or older? You must be at least 18 years of age to get tattooed in the State of Georgia and provide a valid ID.
Which flash design are you interested in?
The designs are priced at their minimum sizes. If you’d like to go bigger in size, the price would adjust accordingly.
Size of the Tattoo in Inches. If you'd like the minimum size, you can put minimum.
Ex: Minimum or 3x3 inches.
Where is the placement of the tattoo?
Ex: Right forearm, back of left calf, etc.
Placement Photo. Please attach a photo where you would like the tattoo to go. This should be a picture of YOUR OWN BODY not a Google image.

If there are other tattoos, scars birthmarks, etc. let it be known! Be sure to take the placement photo of your OWN body where you would like the tattoo to go. Take the photo in good, readable lighting.
