  • Chatham FIELD HOCKEY Scholarship Application Form

    Please complete the form below if you are a Chatham resident and wish to apply for financial assistance for your child’s participation in the Chatham Recreational FIELD HOCKEY program. This is for kids in grade 3rd - 8th. Families who are approved for a scholarship will receive an email containing a registration link and a discount code. The discount code will cover the registration fee for the program. Any equipment/uniform that is needed will be provided or donated. The time commitment will be as follows: 3rd/4th grade will have Sat practice and 8 Sunday games (1/2 in Chatham). 5th-8th grade will have Wed & Sat practice and 8 Sunday games (1/2 in Chatham).

    PLAY IT FORWARD Foundation is committed to safeguarding the privacy of all scholarship applicants. All personal, financial, and supporting information submitted will be treated as strictly confidential and used solely for the purposes of responding to inquiries from interested families and evaluating scholarship eligibility. Access to applicant information is limited to authorized members of the Scholarship Committee and designated board members who are directly involved in the review and selection process. Applicant information will not be disclosed to any third party without prior consent, except where disclosure is required by law.

    By submitting this application and/or accepting support from PLAY IT FORWARD Foundation (the Foundation), the applicant and their parent/guardian acknowledge and agree to the following:

    1. Scholarship Determination
    All scholarship awards are made at the sole discretion of the Foundation based on available funding and established selection criteria. Submission of an application does not guarantee receipt of a scholarship.

    The Foundation, its Board of Directors, committee members, and volunteers shall not be held liable for any decisions regarding the approval, denial, or amount of scholarship awards.


    2. Donated Equipment
     Foundation may provide gently used or donated sports equipment. By accepting such equipment, the applicant and parent/guardian acknowledge that:

    Equipment is provided “as is” without warranties of any kind, either expressed or implied.
    The Foundation does not guarantee the condition, safety, or suitability of the equipment
    It is the responsibility of the parent/guardian to inspect all equipment prior to use.
    The Foundation, its Board members, and volunteers shall not be held responsible for any injury, loss, or damage resulting from the use of donated equipment.

    3. Participation Commitment Statement

    By accepting a scholarship from the Foundation, I understand the importance of consistent participation in my child’s chosen sport. I agree to make every reasonable effort to ensure my child attends scheduled practices, games, and team activities throughout the season.

    I acknowledge that this opportunity is made possible through the generosity of donors and the community, and I will support my child in honoring this commitment to their team.


    4. Participation in Sports Activities
    The Foundation does not operate or control any recreational sports programs. Participation in any sports activity carries inherent risks, including the risk of injury.

    By accepting a scholarship or equipment, the applicant and parent/guardian agree that:

    The Foundation is not responsible for any injuries or incidents occurring during participation in sports programs
    All participation is at the applicant’s own risk

    5. General Release of Liability
    To the fullest extent permitted by law, the applicant and parent/guardian hereby release, waive, and discharge the Foundation and its: Board of Directors, Officers, Committee members, Volunteers, Representatives from any and all claims, liabilities, damages, or expenses arising out of or related to:

    Scholarship decisions
    Equipment distribution
    Participation in recreational activities
    Any other foundation-related activities

    6. Indemnification
    The applicant and parent/guardian agree to indemnify and hold harmless the Foundation and its representatives from any claims or demands arising from the applicant’s participation in programs or use of provided resources.

By submitting this application or accepting financial or equipment assistance from the Foundation, I, as the parent or legal guardian of the Applicant, agree to the terms of the Liability Waiver & Release Statement.
