Pink Pig Birthday Play date Event Waiver & Photo Release
By registering for and attending the Pink Pig Birthday Party event, I acknowledge and agree to the following:
Assumption of Risk
I understand that participation in event activities, including but not limited to inflatables games, attractions, mascot interactions, bubbles, fog and other entertainment activities, involves certain risks of injury, accidents, or illness. I voluntarily assume all risks associated with participation for myself and/or my child(ren).
Release of Liability
I hereby release and hold harmless Partyology Event Planning, event organizers, volunteers, sponsors, venue owners, vendors, staff, and affiliates from any and all claims, liabilities, damages, injuries, losses, or expenses arising out of participation in this event.
Child Supervision
I understand that parents/guardians are responsible for supervising their child(ren) at all times during the event. Event staff and volunteers are not responsible for individual child supervision.
Medical Authorization
In the event of an emergency, I authorize event staff to seek emergency medical treatment for myself and/or my child(ren) if necessary. I understand I am responsible for any medical expenses incurred.
Photo & Video Release
I grant permission for Partyology Event Planning and affiliated partners to take photographs and/or videos during the event and use them for promotional, marketing, social media, website, or advertising purposes without compensation.
Behavior Policy
I understand that unsafe, disruptive, inappropriate, or aggressive behavior may result in removal from the event without notice.
Agreement
By signing below and/or submitting this form, I confirm that I have read, understood, and voluntarily agree to this waiver and release.
