Acknowledgment & Release
I am the parent or legal guardian of the child listed above. I give permission for my child to participate in the inflatable attractions (including moonwalks and other inflatable activities).
I understand that participation in inflatable activities involves physical activity and carries inherent risks, including but not limited to slips, falls, collisions, sprains, fractures, or other injuries.
I acknowledge and agree to the following:
* My child is physically able to participate in these activities.
* My child will follow all posted rules and instructions given by event staff.
* I understand that children who are sick, injured, or have medical conditions that could make participation unsafe should not participate.
* I understand that event staff reserve the right to deny or remove any participant who is acting unsafely or not following the rules.
* I voluntarily assume all risks associated with my child’s participation.
Release of Liability
To the fullest extent permitted by law, I release, waive, and hold harmless Partyology Event Planning, event organizers, volunteers, sponsors, vendors, contractors, and their respective employees, representatives, and agents from any and all claims, liabilities, damages, losses, costs, or expenses arising from or related to my child’s participation, except where caused by gross negligence or willful misconduct.
Medical Authorization
In the event of an emergency, I authorize event personnel to obtain emergency medical treatment for my child if I cannot be reached immediately. I understand that I am responsible for any medical expenses incurred.
