This camp is for Middle School and High School girls.

Camp runs Monday - Friday 8 am - 3pm at the Oakcrest Sports Complex in Harrington, DE.

Emergency Information

Medical Release and Authorization

As the parent/guardian of the named child, I authorize qualified medical personnel to provide first aid, emergency medical care, including necessary treatment or surgery, if I cannot be reached and a delay would risk the child’s health or safety.

I understand that reasonable efforts will be made to contact me as quickly as possible. I accept responsibility for any medical expenses incurred.

I acknowledge that Roots and Wings Academy LLC staff are not medical professionals and will provide basic first aid and supervision until emergency personnel arrive.

This authorization is valid for the duration of the registered program. I confirm that I have provided accurate emergency contact and relevant medical information and will update it as needed.

This consent is given voluntarily to allow necessary care in medical and/or emergency circumstances.

Informed Consent and Acknowledgement

I hereby give my approval for my child/ren’s participation in any and all activities prepared by Roots and Wings Academy LLC during the camp. In exchange for the acceptance of my child/ren’s candidacy by Roots and Wings Academy LLC ., I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Roots and Wings Academy LLC and all its respective officers, agents, and representatives from any and all liability for injuries to my child/ren arising out of traveling to, participating in, or returning from camp sessions and/or associated activities.

In case of injury to my child/ren, I hereby waive all claims against Roots and Wings Academy LLC . including all staff, and, if applicable, owners and lessors of premises used to conduct the camp-related activities and/or events.

Transportation Authorization and Release

I, as the parent/guardian of the named child, give permission for my child to be transported as part of the Roots and Wings Academy LLC Summer Camp program.

I understand that transportation may be provided by Roots and Wings Academy LLC staff or authorized drivers for camp-related activities. I acknowledge that reasonable precautions will be taken to ensure the safety of all participants.

I grant permission for my child to ride in vehicles arranged by the program and agree to instruct my child to follow all safety rules and directions provided by staff.

I understand that I will be notified in advance of any planned off-site trips when applicable.

I release and hold harmless Roots and Wings Academy LLC, its owners, staff, and authorized drivers from liability for injuries or incidents that may occur during transportation, except in cases of gross negligence.

This authorization is valid for the duration of the registered Summer Camp program.

Photo/Media Release

I, as the parent/guardian of the named child on this registration applicationfor Sumer Camp at Roots and Wings Academy LLC, grant permission to Roots and Wings Academy LLC to photograph, record, and/or film my child during participation in the Summer Camp program.

I authorize Roots and Wings Academy LLC to use these images, videos, or recordings for educational, promotional, and marketing purposes, including but not limited to print materials, websites, and social media, without compensation.

I understand that my child’s name will not be used in connection with these materials without additional consent.

This release is granted voluntarily and remains in effect for all media captured during the duration of the Summer Camp program unless revoked in writing.

I waive any right to inspect or approve the final use of such materials and release Roots and Wings Academy LLC from any liability arising from their use.

Payment Policy

A nonrefundable deposit of $50 per week that the child will attend is due upon registration. This deposit will be applied towards the balance for each week. The remaining balance for the week will be due each Monday by 8:00AM. Cost for each week is $200. If your child registers prior to camp starting and attends all 7 weeks, there is a $50 discount per week. 

For all One Day Activity Passes, the $75 fee is nonrefundable and must be paid at the time of registration.

Drop-Off, Pick-Up, and Supervision Policy

Children enrolled in Roots and Wings Academy LLC Summer Camp are under the supervision of staff from 8:00 AM to 3:00 PM each program day.

All children must be signed in and signed out by a parent/guardian or an authorized adult listed on the child’s registration. Identification is required for pick-up.

If a parent/guardian or authorized adult is unable to pick up a child by 3:15 PM, Roots and Wings Academy LLC must be contacted as soon as possible.

A late pick-up fee may be charged for arrivals more than 15 minutes after dismissal time. Repeated late pick-ups may result in removal from the program without refund.

By enrolling, families agree to comply with this policy to ensure the safety and smooth operation of the program.

Behavior Policy

First offense: A verbal warning shall be given to the individual and their parents will be made aware of the incident. Second offense: The individual will be written up and their parents will be made aware of the incident. Third offense: The individual will be expelled from the camp and no refund will be given. This policy is subject to the Roots and Wings Academy LLC staff judgment on a case-by-case basis. In the even of criminal conduct, Roots and Wings Academy LLC will utilize their judgment to contact local authorities and also notify parents of those affected and/or involved.

Cancellation Policy

If you have paid in full and enrolled in this program and are no longer able to participate, please notify rootsandwings302@gmail.com as soon as possible. A formal notice in writing must be made 2 weeks prior to the start of the camp. A refund will be dispersed after the request to cancel. All other requests made after the 2-week deadline will not be guaranteed a full refund.

By completing this application, I agree to be contacted by the camp via phone, text, and email.

Confirmation

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS A PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS A PAPER SIGNATURE.
