Event/Activity: MCWS Middle School Summer Camp (July 24th-26th, 2026)

I, the undersigned parent or legal guardian, acknowledge that my child is voluntarily participating in activities organized by the Muslim Community of Western Suburbs (MCWS) and/or Medina Activity Center (MAC), that involves inherent risks, including, but not limited to physical injury, illness, or property damage.
I hereby RELEASE, WAIVE, DISCHARGE, and agree NOT TO SUE MCWS or MAC, and its board members, trustees, officers, volunteers, employees, and affiliates (collectively referred to as "RELEASEES") from any and all claims, liabilities, demands, actions, or causes of action arising out of any injury, loss, or damage—including serious injury or death—that may occur to my child while participating in these activities or while on the premises where they are conducted.
I understand and agree that this waiver extends to any injuries or damages that may result from the ordinary negligence of MCWS, MAC and its representatives. I also agree that this release and waiver applies to me, my child, and our heirs, assigns, and personal representatives.
Further, I give MCWS/MAC permission to take and use photos/videos of my child for promotional purposes.

CAMP CAVELL - PARTICIPANT AUTHORIZATION

People participating at programs at Camp Cavell are required to complete our Participant Authorization.Please initial each box for authorization:
ACTIVITY RELEASE:

I (my child) have permission to engage in all prescribed camp activities, except as noted by me or an examining physician. In case of serious injury or illness, the emergency contact person will be called immediately for their decision on medical treatment. If parents or emergency contact persons are not available, we will use our best judgment as to what course of action to pursue and will continue to attempt contact. Camp Cavell Conservancy will not be responsible for any costs incurred because of illness or injury. If you (your child) become ill or have a minor injury, we will treat them at camp and contact emergency contacts or guardians if care to a greater extent or more information is needed.I understand that I (my child) may have the opportunity to receive an introductory lesson in technical tree climbing, kayaking, horseback riding, team building or other adventure activities that have inherent risks. Campers would participate only after the evaluation of the readiness of each camper, with the proper safety equipment and trained staff. Introductory lessons would include safety orientation and limit the campers to a safe height or distance appropriate for their age and abilities. Campers are responsible for following the safety rules and directions of the staff.
