I, _______________________________________ fully understand that I must abide by all of the requirements of being out on bond, as set forth by Action Bail Bonds.

1. I must check in using the Bail Bond Check-in app. once per week.
2. Obtain an attorney, if needed before your case comes to court and notify our office.
3. Notify our office in writing of any employment or address changes.
4. Notify our office and receive written approval if you must travel out of town.
5. Notify our office upon receipt of court settings.
6. I must bring a copy of any dismissal or final judgment I receive in this case before I am relieved of my responsibilities in this Bail Bond Release Agreement.
7. I must not commit any act which shall constitute reasonable evidence of my intention to cause a forfeiture.

I also understand that if I or my Co-Signer/Indemnitor provides any information that is false, or if I do not follow the above listed requirements and if for any reason I should be arrested again, Action Bail Bonds may sign off of my bond at any time without notice. I also understand that if Action Bail Bonds signs off of my bond, Action Bail Bonds will refund -0- dollars. I authorize you to obtain my credit report if necessary to verify any information that I provide you.

I understand that the premium owing or paid is fully earned upon the defendant's release from custody, and the fact that the defendant may have been improperly arrested, re-arrested, the case dismissed, or the bail reduced shall not obligate the return or forgiveness of any portion of the premium except as otherwise provided by applicable law (if any) as stated in an addendum attached to the Bail Bond Application and Agreement.

My signature on this legal document gives Action Bail Bonds as surety, all rights to GO OFF OF MY BOND WITHOUT REFUND. I acknowledge receipt of a copy of this document on the date listed below.
