Science and Social Studies Program 

• Program will be held on Wednesdays from 9:30am-1:00pm
• Location: Trinity Lutheran Church (3973 N Cedar Ave Fresno CA 93726)
• TK-6th: $160 per session (4 weeks)
• Early Learners Drop-off (PreK/TK): $140 per session (4 weeks)
• Early Learners Co-op (PreK/TK): Free (limited spots)
• Program held from August 19, 2026 through May 26, 2027 with days off for Thanksgiving, Winter Break, and Spring Break. (Please see 2026-2027 EM Calendar for exact dates).

Medical Release

I, {primaryParentguardian}, the parent or legal guardian of {studentName}, give permission to Enlightened Minds for any and all medical attention to be administered to my child in the event of an emergency, when the need for treatment is clear, and when all efforts have been made to contact me. By signing below I also assume the financial responsibility of payment for any and all expenses incurred for treatment.

Waiver of Liabiity 

 I, {primaryParentguardian}, parent or legal guardian of {studentName} give my full permission for my child to participate in classes and activities at Enlightened Minds. In allowing my child to participate in classes and activities at Enlightened Minds, I fully and completely release, discharge, and hold harmless Enlightened Minds, and its employees and volunteers from any and all claims, actions, causes of action, and damages of any kind whatsoever, including but not limited to claims for personal injury, death, or property damage, arising out of my child's participation in classes and activities at Enlightened Minds.

I acknowledge and recognize there are inherent risks involved in such activities and I assume the risk of any injury sustained while at Enlightened Minds. I agree to indemnify, defend and hold harmless Enlightened Minds and its employees and volunteers from any and all claims arising out of my child's participation in any class or activity at Enlightened Minds, even if such claim arises as a result of a negligent act or omission of Enlightened Minds or its employees and volunteers.

Media Authorization and Release

Enlightened Minds requests permission to photograph and/or record your child while participating in classes and/or activities and for the resulting images and/or recordings to be used for internal newsletters, social media marketing, and/or training purposes.

By signing below, I hereby release and hold harmless Enlightened Minds and its agents and employees from any all claims arising from the use of photographs and/or recordings.

Technology Policy

 I, {primaryParentguardian}, parent or legal guardian of {studentName} understand that cellphones, tablets, smartwatches, laptops, and similar devices are not allowed to be used for entertainment purposes while my child is on campus. Cellphones and smartwatches should only be used to communicate with parent or legal guardian and should be placed on silent during my child's time at the learning center to foster a focused and distraction-free learning envrionment and to ensure student safety and well-being for all students on campus. Tablets, laptops, and similar devices should only be brought and used for educational purposes when my child will be doing academic work.

I understand that if my child brings any of these devices to campus, I am responsible to share the expectation of this policy with them. I understand that if my child has a hard time following this policy, Enlightened Minds may ask that my child not bring the device or another alternative be followed.

Enrollment: I understand Enlightened Minds offers a year long Science & Social Studies Program, and if I decide that my child will no longer attend, I will extend the courtesy of giving Enlightened Minds sufficient notice, so that their spot in the class can be made available to a student on the waitlist.

Missed Work: I understand that Enlightened Minds will provide any worksheets missed when my student is absent. I further understand that it is my responsibility to pick up any missed worksheets at the sign in tables the week my student returns to class.

Communication: Communication will be made via email, phone, and text messages. If I have any questions about the program or activities I will reach out via email to info@enlightenedminds.net or by phone/text to (559) 569-1375.

Drop off/Pick up: I will arrive on time and pick up on time. I will notify Enlightened Minds if my child will be absent, late, or if I need to request an early pick-up.  

Water, Lunch, and Snacks: I will be sure to send my child with a water bottle and lunch, and if needed snacks, so they are ready for a 3.5 hour day of instruction and activities. I will make sure to communicate any allergies or sensitivities which may affect my student or the class.

Holidays/Emergency/Class Cancellations: I am aware that some of the class days may fall on a holiday and that some class days may close due to holidays. In the event of closure due to emergency or other unforeseen circumstances, I am aware that there are no make-up days.

Behavior & Conduct: I will do my best, within my abilities as the parent/guardian, to encourage my student to be prepared for class and ready to participate with a positive attitude. I will instruct my child to show respect for Enlightened Minds instructors, helpers, and other students. I will share with them the expectation that they follow the rules, which includes instructing them to remain with their group and not wander off into other areas of campus. I understand that if there are ongoing behavioral issues or other concerns regarding my child's participation and/or behavior, Enlightened Minds may come to the conclusion that the program is not a good fit for my child, and that they may be dismissed from further instruction. 

Class Fee: I understand that I am responsible for my child's class fee, which must be received before the first of the month and that they may not be allowed to attend class if the payment has not been received and/or confirmed.

Charter School Students: I understand that it is my responsibility to submit the service order request for the session fee in a timely manner. This usually requires that I submit the order two (2) weeks prior to the start of class, in order for Enlightened Minds to receive my approved certificate before class begins. If Enlightened Minds does not receive my approved certificate by the first of the month, I understand that my student may not be allowed to attend class and their spot will be made available to a student on the waitlist unless I private pay for the session.

I also understand that it is my responsibility to be informed of my charter school's specific guidelines regarding service orders. I understand that if I request services from my charter for use at Enlightened Minds, and I do not utilize the services, that my charter school may not issue me a refund for unused services, and that it is not the financial responsibility of Enlightened Minds to reimburse me for unused services.

Property Damage: I understand that if my child is found to be responsible for any damage to the property where classes are held, I may be financially responsible for the damages.

Refunds/Credits: I understand that there are no refunds or credits for missed days due to illness, vacation, or any other circumstance. If there are closures due to unforeseen events, I understand that we may not have the option for a make-up day.
