Anonymous Feedback Submission

Thank you for taking the time to share your feedback. Your submission is anonymous and will be sent directly to the CAHA Executive Board of Directors. Please do not include any personal identifying information in the content of your submission or in any documents you upload.

Because this form is anonymous, you will not receive a direct response. However, the Board values honest and constructive feedback and will review, evaluate, and discuss every submission received.

Important – SafeSport Concerns:
While you may submit general feedback regarding SafeSport through this form, if you are reporting a potential SafeSport violation, misconduct, abuse, or any concern requiring investigation or follow-up, we strongly encourage you to report it directly through the appropriate SafeSport reporting process. Reporting through the official SafeSport channels allows trained personnel to respond promptly, gather necessary information, protect confidentiality, and take appropriate action.
