The Hogue Foundation (THF) – Football & Friends Day

I, the undersigned parent or legal guardian, give permission for my child to participate in The Hogue Foundation’s Football & Friends Day event (the “Event”).

1. Assumption of Risk

I acknowledge that participation in sports and recreational activities involves inherent risks, including but not limited to physical injury. I confirm that my child is physically able to participate and has no medical condition that would prevent safe involvement.

I voluntarily assume all risks associated with my child’s participation.

2. Medical Responsibility

I understand that The Hogue Foundation does not provide medical insurance coverage for participants. I accept full responsibility for any medical expenses that may arise from my child’s participation.

3. Liability Waiver & Release

On behalf of my child and our family, I agree to release, waive, and hold harmless The Hogue Foundation (THF), its officers, directors, staff, volunteers, partners, and affiliates from any and all claims, liabilities, damages, or expenses (including attorney’s fees) arising out of or related to participation in the Event.

4. Conduct Responsibility

I understand that my child is expected to follow all instructions and behave appropriately. I accept responsibility for my child’s actions during the Event.

5. Media Release

I grant permission for The Hogue Foundation to photograph and/or record my child during the Event and to use these images or videos for promotional, marketing, and informational purposes, including social media, website, and print materials. No compensation will be provided.

I am the parent or legal guardian of the participant and have the legal authority to sign on their behalf. I have read and fully understand the Liability Release Waiver and all related terms. I have explained the risks of participation to my child, and we voluntarily agree to participate. I understand the risks involved and knowingly assume those risks on behalf of my child. I am signing this agreement voluntarily, without coercion, and with full intent to be legally bound. I consent to the use of my electronic signature as the legally binding equivalent of my handwritten signature.

This waiver shall remain in effect for this event and any related activities conducted by The Hogue Foundation (THF).
