  • I certify that the information provided in this application is true, complete, and accurate to the best of my knowledge. I understand that any misrepresentation, falsification, or omission of material facts may result in disqualification from consideration for employment or, if employed, termination of employment.

    I understand that this application constitutes Phase One of the employment process. If selected to proceed, I will be required to complete additional documentation, which may include, but is not limited to, authorizations for background checks, motor vehicle records (MVR) reviews, drug and alcohol testing, and other employment-related screenings in accordance with applicable federal and Missouri laws.

    I authorize Capital Technologies LLC to investigate all statements contained in this application and to contact my prior employers, references, and any other relevant parties for the purpose of verifying the information provided. I hereby release all such persons and entities, as well as Capital Technologies LLC, from any and all liability arising from the investigation or disclosure of such information, to the fullest extent permitted by law.

    I understand that no representative of Capital Technologies LLC has the authority to enter into any agreement for employment for a specified period or to make any agreement contrary to the foregoing, unless such agreement is in writing and signed by an authorized representative of the company.

    I acknowledge that this authorization does not permit the release or use of disability-related or medical information in any manner prohibited by the Americans with Disabilities Act (ADA) or other applicable federal or state laws.

    I understand that, as part of the application and hiring process, Capital Technologies LLC may require a consumer report and/or investigative consumer report, which may include information regarding my creditworthiness, criminal history, driving record, or other background information, as permitted by Missouri law. I understand that, if such reports are requested, I will be provided with a separate written disclosure and authorization form in accordance with the Fair Credit Reporting Act (FCRA) and applicable state laws. I further understand that the existence of a criminal conviction or adverse credit history will not automatically disqualify me from employment, and that any such information will be considered in accordance with applicable law.

    I understand that employment with Capital Technologies LLC may be contingent upon successful completion of a pre-employment drug and/or alcohol screening, and I consent to such testing in accordance with Missouri law and company policy. I further understand that refusal to submit to such testing or a confirmed positive result may result in withdrawal of a conditional offer of employment or termination, where permitted by law.

    I acknowledge that, depending on the position and project requirements, I may be required to comply with federal, state, local, or customer-specific safety standards, including but not limited to OSHA regulations and site-specific safety rules. Failure to comply with such requirements may result in disciplinary action, up to and including termination.

    I understand that, in compliance with federal law, all individuals hired will be required to verify their identity and eligibility to work in the United States and complete the required employment eligibility verification documentation at the time of hire.

    I further acknowledge and understand that employment with Capital Technologies LLC is on an “at-will” basis, which means that either the employee or the employer may terminate the employment relationship at any time, with or without cause and with or without notice, subject to applicable law.
