This Revival Room Organization Partnership Agreement is entered into by and between Assurgent Healing, the provider of Revival Room, and the organization or Ministry/Team organization identified in this form.
By completing this agreement, the authorized representative confirms that they have authority to enroll the organization listed above and agree to the terms below.
1. Purpose of Revival Room Revival Room is an organization wellness and accountability platform designed to support emotional awareness, leadership reflection, Ministry/Team team check-ins, and trend-level visibility for organization leadership.
Revival Room is intended to support healthier Ministry/Team culture, early awareness of emotional strain, and structured leadership support. It is not a replacement for therapy, crisis intervention, medical care, pastoral counseling, or emergency support.
2. Organization Access and Implementation Access to Revival Room is granted only after completion of this agreement and review of the submitted organization implementation information.
Onboarding information is sent within 24–72 hours after information review for proper organization implementation.
The organization agrees to provide accurate information regarding its Ministry/Team structure, primary contacts, administrators, and intended Ministry/Team teams.
3. Authorized Organization Use Revival Room access is limited to the organization or Ministry/Team organization listed in this agreement.
The organization agrees not to share access codes, administrative access, login credentials, or internal platform access with any other organization, campus, Ministry/Team organization, business, or outside group unless approved in writing by Assurgent Healing.
Each separate organization, campus, or organization may require its own enrollment unless otherwise agreed upon in writing.
4. Ministry/Team and Member Access The organization is responsible for inviting only appropriate leaders, staff, volunteers, or members into its Revival Room organization environment.
The organization is responsible for managing who receives access codes and ensuring that access is used only for the intended organization community.
The organization agrees to notify Assurgent Healing if access needs to be changed, restricted, or reviewed.
5. Confidentiality and Trend-Level Visibility Revival Room is designed to protect individual privacy while giving leadership access to trend-level insight.
Leadership views are intended to show patterns, participation, and Ministry/Team-level trends. Individual private reflections, journals, or disclosures are not intended to be displayed to leadership unless a user chooses to share information through available sharing features.
The organization agrees not to use Revival Room to pressure, punish, monitor, discipline, or retaliate against individuals based on wellness participation or emotional check-in patterns.
6. Appropriate Use The organization agrees to use Revival Room in a respectful, lawful, and ethical manner.
The organization agrees not to misuse the platform, attempt to access unauthorized data, share false information, upload harmful content, interfere with platform security, or use the platform in a way that violates the rights, privacy, or dignity of others.
7. AI-Supported Reflection Revival Room may include AI-supported reflection tools designed to offer general support, prompts, and perspective.
AI-supported content is not professional medical, clinical, legal, financial, or emergency advice. Users should seek appropriate professional support when needed.
All plans include standard AI-supported reflection usage. If usage materially exceeds the standard amount included in the selected tier, additional usage fees may apply and will be communicated clearly.
8. Billing and Free Trial The selected plan begins with a 30-day free trial.
Unless canceled before the trial period ends, the organization will be billed annually according to the selected plan.
Current plan pricing is based on the selected organization tier and Ministry/Team capacity listed at enrollment.
The organization understands that failure to complete onboarding requirements does not automatically cancel the trial or subscription. Cancellation must be completed through the appropriate billing process or by contacting Assurgent Healing before the trial ends.
9. Cancellation The organization may cancel during the 30-day free trial to avoid annual billing.
After annual billing occurs, access remains active for the paid annual term unless otherwise stated in writing.
Refunds, if any, are handled according to Assurgent Healing's stated policies and any applicable written agreement.
10. Platform Availability and Updates Assurgent Healing may update, improve, modify, or temporarily interrupt Revival Room for maintenance, security, performance, or feature development.
Assurgent Healing will make reasonable efforts to support platform availability, but does not guarantee uninterrupted or error-free access.
11. Intellectual Property Revival Room, including its design, structure, content, features, workflows, branding, and related materials, remains the intellectual property of Assurgent Healing or its authorized owners.
The organization receives limited access to use Revival Room for its enrolled organization community. This agreement does not transfer ownership of the platform, materials, branding, content, or technology.
The organization agrees not to copy, reproduce, resell, redistribute, reverse engineer, or create derivative works from Revival Room without written permission.
12. Data and Privacy The organization agrees to use Revival Room in a manner that respects privacy and confidentiality.
Assurgent Healing may collect and process information necessary to provide platform access, support onboarding, maintain security, manage billing, and improve services.
The organization is responsible for informing its leaders, staff, volunteers, or members how Revival Room is being used within its Ministry/Team context.
13. No Emergency Use Revival Room is not an emergency response system.
If a user is experiencing a mental health emergency, safety risk, abuse concern, or immediate crisis, they should contact emergency services, crisis support, local authorities, a licensed professional, or appropriate organization safety leadership.
14. Termination or Access Restriction Assurgent Healing may restrict, suspend, or terminate access if there is misuse of the platform, unauthorized sharing, nonpayment, violation of this agreement, or conduct that creates risk to users, the organization, or Assurgent Healing.
15. Agreement Acknowledgment By signing below, the authorized representative confirms that:
- They are authorized to enter this agreement on behalf of the organization listed in this form.
- The information provided is accurate to the best of their knowledge.
They agree to the terms of this Revival Room Organization Partnership Agreement.
- They understand that access is granted after agreement completion and information review.
- They understand onboarding information is sent within 24–72 hours after review for proper organization implementation.
