  • Documentation to support scholarship requests for family members is required with submission of this form and may include but is not limited to the following:
    • Legal Spouse - valid marriage license or certificate
    • Biological Child – birth certificate or financial support records (tax returns, child support records, etc.)
    • Adopted Child – certified copy of Adoption Decree, an amended birth certificate listing you as the parent, or official documentation from a child welfare agency showing the placement and finalization of the adoption.
    • Stepchild – valid marriage license showing your marriage to the child's biological parent and the child's birth certificate listing the biological parent you are married to; or affidavits from school or medical providers; or financial support records such as tax returns

  • The following policies apply to all employees participating in the Educational Assistance Program (EAP) or enrolled in Goodwill Technical College (GTC):
    • Supervisor Consultation: Staff members are responsible for meeting with their immediate supervisor prior to enrolling in courses to discuss potential scheduling impacts.
    • Schedule Modifications: Any request for a modified work schedule to accommodate class attendance is subject to departmental needs and supervisor's approval. Participation in the EAP or admission to GTC provides the financial means for education but does not guarantee a modified or flexible work schedule.
    • Off-the-Clock Requirement: At no time may an employee attend classes, labs, or mandatory college functions while "on the clock." All coursework must be completed during unpaid breaks or outside of the employee's designated working hours.
    • Performance Standards: Employees are expected to maintain their standard of job performance. Participation in the EAP or GTC scholarship program does not excuse any decline in productivity or failure to meet workplace responsibilities.
