Before you start, 

Complete sections A-E of your application page included in the RFP. Please ensure you have your own copies saved of the grant application and any required attachments. If you wish to save progress of application package uploads, it is encouraged to create a free jotforms account.

This jotform is created for ease of submitting your application package with the required attachments. It does replace the application package included in the RFP.

The following section is for uploading sections A-D in your Application Package. These sections are included in the Request For Proposal (RFP). You may upload the entire RFP with sections A-D completed. Make sure the following sections are completed before uploading your file, 

A. Organization Name and Contact 

B. Project Overview and Summary 

C. Project Narrative and Timeline

D. Grant Budget

The following section is for uploading any required attachments in section E of your Application Package. Make sure to submit the following attachments,

• Letter(s) of Support 
• Project Location Map
• WSA site assessments 
• Proposals/Quotes 
• HOA/Community Project Authorization Form - if applicable 
• Current Designs - if applicable
• Transmittal Letter - if applicable
