I the undersigned have read and understand the policies and procedures of C&C Ballet Academy and agree to abide by them as long as my child is enrolled. Student and parent/guardian understand and expressly assumes all risk of bodily injury occurring as a result of contact with other students, instructors, structures, equipment and other objects located on the studio property, or the student's physical condition or limitations. Student waives all claims arising out of any and all activity associated with C&C Ballet Academy, whether caused by negligence, breach of contract, or otherwise, and whether for bodily injury, property damage or loss or otherwise, which student may ever have against C&C ballet Academy, it directors, employees and administrators. I gave C&C Ballet Academy permission to use photos of my student for promotional purposes in either print or electronic formats.

C&C Ballet offers a family discount of 10% off the first class for each member in the family. Dancers wishing to take more than one class will receive each additional class offered at a 50% discount. The highest priced class will be full price. The 10% family discount cannot be combined with the 50% discount.
