Answer all or some of the questions.
We encourage you to write as if the person reading this believes you, because we do.
Question 1: Why do you think you were targeted?
Tell us about the type of discrimination you experienced. For example: I believe I was targeted because of my age, because I was the only person over 40 and my boss treated me significantly worse than my peers and teammates.
Question 2: What were your earliest warning signs that something was off?
Think back to before you were certain it was discrimination. What were the subtle comments that felt off or first signs that you were being treated differently? This might be something you initially explained away, told yourself you were overreacting to, or weren't sure how to name.
Question 3: What changed to help you recognize the discrimination you were experiencing for what it was?
This can be the moment you realized it was discrimination or the moment that makes you feel certain what you experienced was discrimination when you think about what happened now.
Question 4: What did you do when you realized it was discrimination. Is there anything you'd do differently?
There are no wrong or right answers here. We know how common it is for people to blame themselves and we mainly want people to know that if they didn't do everything right, that's ok and they're not alone.
Question 5: How did discrimination impact you: personally, professionally, or financially?
We recognize this is a very personal question. Feel free to share as little or as much as you're comfortable sharing.
Question 6: Is there any advice or lesson learned that you'd like to share with others?

Thank you for making a difference
We know reliving this is difficult, so we appreciate you sharing and being the reminder someone needs that they're not the first, they're not the only and they're not alone.
