1. In consideration for the transport of my pet, ____________________, out of or off of St. Thomas, USVI, under any and all circumstances, including exigent circumstances (e.g., natural disasters, emergencies) and/or any other circumstances, I hereby release, waive, discharge and agree not to initiate any legal action against Pets With Wings, Inc. and/or its volunteers (hereinafter referred to as "PWW"). I release PWW from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or relating to any loss, damage or injury, including death, that may be sustained by my pet(s), or to any property belonging to me, whether caused by PWW negligence or otherwise, while in transit to or from the US Virgin Islands and other US destinations, or in any place or places connected with a flight off island or evacuation.

2. I am fully aware of risks and hazards associated with a flight off the island or during an evacuation. I voluntarily assume full responsibility for any risk of loss, property damage, including death of my pet, that may be sustained by me, or any loss or damage to property owned by me, whether caused by the negligence of PWW or not.

3. I further hereby agree to indemnify and hold harmless PWW, from any loss, liability, damage or costs they may incur whether caused by the negligence of PWW or otherwise connected with the transportation of my pet(s).

In signing this release and waiver, I acknowledge and represent that: I have read the foregoing release and waiver; I fully understand this release and waiver I sign it voluntarily as my own free act and deed. No oral representation, statements or inducements, apart from the foregoing release and waiver, have been made; I am at least eighteen (18) years of age and fully competent; and I execute this release and waiver for full, adequate and complete consideration fully intending to be bound by same.
