The LIVE (LGBTQ+, Inclusivity, Visibility & Empowerment) Program invites you to join us for a powerful day of celebration, connection, and advocacy for LGBTQ+ mental wellness. Together, we'll unite diverse voices from across California to strategize, connect, and mobilize for collective action.

Here's what you can expect:

Photo, Video and Liability Release Form: I give permission to Mental Health America of California, fiscal agent for Stonewall Alliance Center, MoPride, Reach LA, North County LGBT Resource Center, Asian Mental Health Project, AIDS Healthcare Foundation Los Angeles, Drag Story Hour, San Francisco AIDS Foundation, Library Foundation San Diego, AIDS Healthcare Foundation San Diego, Vietnamese-American Arts & Letters Foundation (VAALA), Viet Rainbow Of Orange County (VROC), Sacramento LGBT Community Center and Behavioral Health Services Oversight and Accountability Commission (Commission for Behavioral Health) to use photographic pictures and/or video images. 

I understand this may include posting the photographs and/or videos on web sites and social media sites, and including them in printed and electronic materials. I waive the right to inspect or approve the finished product or products or the copy or printed matter that may be used in connection therewith or the use to which it may be applied. I release Mental Health America of California, fiscal agent for Stonewall Alliance Center, MoPride, Reach LA, North County LGBT Resource Center, Asian Mental Health Project, AIDS Healthcare Foundation Los Angeles, Drag Story Hour, San Francisco AIDS Foundation, Library Foundation San Diego, AIDS Healthcare Foundation San Diego, Vietnamese-American Arts & Letters Foundation (VAALA), Viet Rainbow Of Orange County (VROC), Sacramento LGBT Community Center Behavioral Health Services Oversight and Accountability Commission (Commission for Behavioral Health) from any liability and any claims for libel or invasion of privacy.

 

I have read the above authorization, release and agreement. This release shall be binding upon me and my heirs, legal representatives and assigns.

Waiver of Liability: I hereby, on behalf of myself, my heirs, personal representatives, assigns, and agents do forever, release, waive, discharge and covenant not to sue, Mental Health America of California, fiscal agent for Stonewall Alliance Center, MoPride, Reach LA, North County LGBT Resource Center, Asian Mental Health Project, AIDS Healthcare Foundation Los Angeles, Drag Story Hour, San Francisco AIDS Foundation, Library Foundation San Diego, AIDS Healthcare Foundation San Diego, Vietnamese-American Arts & Letters Foundation (VAALA), Viet Rainbow Of Orange County (VROC), Sacramento LGBT Community Center, Behavioral Health Services Oversight and Accountability Commission (Commission for Behavioral Health) and/or its respective Board members, executives, administrators, officers, directors, agents, employees, contractors and volunteers from any and all claims, causes of action, or other judicial proceedings, costs, losses, expenses, damages and liabilities, including attorneys’ fees arising out of or related to, directly or indirectly, all known or unknown personal injuries or property damage or any harm resulting from my participation to the extent permitted by law.

Assumption of Risks: I, hereby, on behalf of myself, my heirs, personal representatives, assigns, and agents voluntarily assume full responsibility for any risk of bodily injury, property damage or any harm arising out of or related to the organization sponsored trip to the extent permitted by law.

Indemnification and Hold Harmless: I, hereby, on behalf of myself, my heirs, personal representatives, assigns, and agents agree to indemnify and hold harmless Mental Health America of California, fiscal agent for Stonewall Alliance Center, MoPride, Reach LA, North County LGBT Resource Center, Asian Mental Health Project, AIDS Healthcare Foundation Los Angeles, Drag Story Hour, San Francisco AIDS Foundation, Library Foundation San Diego, AIDS Healthcare Foundation San Diego, Vietnamese-American Arts & Letters Foundation (VAALA), Viet Rainbow Of Orange County (VROC), Sacramento LGBT Community Center,Behavioral Health Services Oversight and Accountability Commission (Commission for Behavioral Health) and/or its respective Board members, executives, administrators, officers, directors, agents, employees, contractors and volunteers from any claims, causes of action, or other judicial proceedings, costs, expenses, damages and liabilities, including attorneys’ fees, brought as a result of my participation in the organization sponsored event to the extent permitted by law.
