Informed Consent and Acknowledgement
Program Overview
Brave Tween Labs is a skills-based group designed to support tweens in building confidence, connection, and practical tools through creativity, conversation, mindfulness, and nature-based experiences. This program is not therapy and does not replace mental health treatment or clinical care.
Confidentiality
Participants will be encouraged to respect one another’s privacy and create a safe, supportive environment. While confidentiality will be emphasized within the group, it cannot be guaranteed, and parents or guardians agree to support these expectations at home.
Assumption of Risk and Liability Waiver
I understand that participation may include physical activities such as movement and outdoor experiences including hiking. I voluntarily assume all risks associated with participation. I release and hold harmless Brave Labs, its facilitators, and affiliates from any liability, injury, or damages that may occur.
Consent for Participation
I give permission for my child to participate in Brave Tween Labs. I understand the nature of the program and agree to allow my child to engage in all planned activities.
