Patient Policies, Financial Agreement, Telehealth & Communication Policies

Please review all policies carefully prior to booking. By scheduling an appointment, completing intake paperwork, submitting payment, or participating in services through Optimize/Navara Health, you acknowledge and agree to the policies outlined below.
APPOINTMENT BOOKING & DEPOSITS
A booking deposit is required to reserve all appointments and consultations.
If the booking deposit does not cover the full service amount, the remaining balance will be invoiced separately via email and/or text message.
Remaining balances must be paid in full prior to:
appointment confirmation
laboratory orders
prescription processing
work/school notes
medical documentation
treatment plan initiation
Appointments are not considered confirmed until:
all required intake forms are completed
all required consents are signed
payment has been received in full
requested records or labs have been submitted when applicable
After booking, patients will receive communication from our team regarding:
required forms
intake paperwork
consent documents
remaining balances due
laboratory instructions if applicable
CANCELLATION & NO SHOW POLICY
Appointments cancelled or rescheduled less than 24 hours prior to the scheduled appointment time will forfeit the booking deposit.
Missed appointments, repeated late cancellations, or repeated no-shows may require prepayment for future appointments or dismissal from the practice.
TELEHEALTH & STATE ELIGIBILITY
Patients must be physically located in a state where the provider is licensed at the time of the appointment.
Optimize/Navara Health currently offers telehealth services in:
Texas
Colorado
Florida
Oklahoma
Iowa
Vermont
Virginia
Washington
Connecticut
Additional states may be added in the future.
Patients are responsible for accurately reporting their physical location during telehealth visits.
CONTROLLED SUBSTANCE POLICY
Testosterone is currently the only controlled substance prescribed by this practice.
Testosterone prescribing is limited to eligible Texas patients only and requires:
an established provider-patient relationship
compliance with Texas and federal prescribing laws
ongoing monitoring
annual in-office evaluation at least once every 12 months
Controlled substances will not be prescribed across state lines outside of Texas.
Optimize/Navara Health does not prescribe:
chronic opioid medications
stimulant medications
benzodiazepines
sleep medications
long-term psychiatric medication management
We do not manage insulin-dependent diabetes or prescribe insulin therapy.
CONSULTATION & MEDICAL ADVICE POLICY
All services require medical evaluation and provider approval.
Until a formal provider-patient relationship has been established, Optimize/Navara Health cannot provide:
medical advice
diagnosis
prescription recommendations
treatment recommendations
work notes
disability/FMLA paperwork
custom forms
laboratory interpretation outside of booked appointments
Treatment eligibility is determined only after review of:
medical history
medications
symptoms
laboratory testing
contraindications
provider assessment
Laboratory testing may be required before initiation or continuation of treatment.
Treatment approval is never guaranteed.
LABORATORY TESTING POLICY
Optimize/Navara Health offers self-pay laboratory testing through third-party laboratory partners including Quest Diagnostics using discounted cash-pay pricing through platforms such as Fullscript and Rupa Health.
Optimize/Navara Health does not directly bill insurance for laboratory testing.
Patients may:
purchase self-order laboratory panels
request custom laboratory panels after consultation
obtain laboratory requisitions
request required laboratory lists to provide to their PCP
If patients choose to utilize insurance through Quest Diagnostics or another laboratory, Navara Health is not responsible for:
denied claims
insurance balances
unexpected bills
laboratory billing disputes
coverage determinations
Patients are solely responsible for verifying insurance coverage.
SELF-ORDER LAB POLICY
Self-order laboratory testing does not include:
diagnosis
interpretation
medical advice
treatment recommendations
prescription management
Once laboratory results are finalized, patients will receive notification through the laboratory platform.
It is the patient's responsibility to:
log into the booking platform
schedule a follow-up appointment
book a laboratory review consultation if interpretation is desired
Optimize/Navara Health is not responsible for reviewing or interpreting laboratory results unless a follow-up appointment has been scheduled and completed.
PRESCRIPTION & REFILL POLICY
Prescription approval is based on provider discretion, safety review, laboratory results, medical history, and treatment compliance.
Refills are not guaranteed.
Patients are responsible for requesting refills early enough to avoid interruption in therapy.
All refill requests must be submitted through the patient portal.
Please allow adequate processing time for refill requests.
Many medications are compounded and shipped through licensed 503A compounding pharmacies.
Delays may occur due to:
pharmacy processing times
medication shortages
shipping carrier delays
high medication demand
Optimize/Navara Health is not responsible for pharmacy or shipping delays.
WEIGHT LOSS PROGRAM POLICIES
Patients participating in Semaglutide or Tirzepatide programs are required to submit monthly weight updates, side effect updates, and progress information before refill approval.
Failure to submit updates may delay refill processing or continuation of treatment.
Patients requesting Novo Nordisk Direct, Lilly Direct, or retail pharmacy GLP-1 prescriptions must maintain active medication management membership services.
Membership fees apply separately from medication costs billed by retail pharmacies or manufacturers.
PRIOR AUTHORIZATION POLICY
Prior authorization services are available for an additional administrative fee.
Current prior authorization fee: $99 per submission.
Payment of a prior authorization fee does not guarantee insurance approval or medication coverage. Insurance decisions are determined solely by the patient's insurance carrier.
COMMUNICATION POLICY
Patients may communicate with the practice through:

OptiMantra mobile app
Spruce HIPAA-compliant messaging platform
Messages are generally answered within 24–48 business hours. Response times may vary depending on patient volume, weekends, holidays, and provider availability.
Do not use standard SMS texting for protected health information outside of HIPAA-compliant platforms.
Medical emergencies should never be handled through portal messaging, text messaging, voicemail, or email.
If you are experiencing a medical emergency:

Contact local emergency services
PAYMENT POLICY
All patients must maintain an active card on file.

Payment processing fees:
Credit cards: 3%
Cherry Financing: 5%
To avoid processing fees, patients may pay via Zelle or Apple Pay using:

Failure to maintain valid payment information may delay treatment, appointments, prescriptions, or refill processing.
MEMBERSHIP POLICY
Membership fees cover medical management services and ongoing provider oversight.
Membership fees do not guarantee:
medication approval
insurance coverage
prescription approval
laboratory coverage
unlimited access
emergency care services
Membership fees are non-refundable once services have been initiated.
COMPOUNDED MEDICATION DISCLOSURE
Many medications prescribed through Optimize/Navara Health may be compounded medications prepared by licensed 503A compounding pharmacies.
Compounded medications are not FDA-approved in the same manner as commercially manufactured medications.
Patients acknowledge understanding of:
compounded medication use
potential shortages
pharmacy processing delays
individualized formulations
off-label prescribing when applicable
FINANCIAL RESPONSIBILITY
Patients are financially responsible for:
all booked services
laboratory testing
medication costs
pharmacy shipping fees
financing fees
administrative fees
missed appointment fees
insurance balances related to outside laboratory processing
ACKNOWLEDGMENT
By booking an appointment, completing intake paperwork, submitting payment, participating in telehealth services, or receiving treatment through Optimize/Navara Health, you acknowledge that you have reviewed, understand, and agree to all policies outlined above.
All sales and services are final unless otherwise required by law.
