Sports Camp Liability Waiver & Release of Claims — Mann Up Athletics at Chartwell School
Assumption of Risk: I understand participation in Mann Up Athletics Sports Camp involves physical activity and inherent risks, including but not limited to: injury, muscle strains/sprains/fractures, head injuries/concussions, heat-related illness, equipment-related injuries, contact with other participants, and risks arising from my actions, others’ actions, or facility conditions.
Release of Liability: I, for myself and on behalf of my child, our heirs, assigns, and next of kin, release and hold harmless Mann Up Athletics, its owners, coaches, trainers, staff, contractors, affiliates, and Chartwell School, its administrators, employees, and representatives, from any and all liability for injury, illness, death, property damage or loss, and negligence, to the fullest extent permitted by law.
Medical Authorization: I certify that the participant is physically fit to participate. I authorize Mann Up Athletics staff to administer basic first aid and seek emergency medical treatment as needed. I accept financial responsibility for any resulting medical care.
Communicable Illness Acknowledgment: I understand participation may expose my child to communicable illnesses, including COVID-19.
Code of Conduct: I agree the participant will follow all coaching instructions, treat staff and athletes respectfully, and use equipment safely. I understand unsafe or disruptive behavior may result in removal from camp without refund.
Photo and Media Release: I grant permission for photos or videos of the participant to be used for marketing, social media, website, and promotional materials.
Indemnification: I agree to indemnify and hold harmless Mann Up Athletics and Chartwell School from any claims arising from participation.
Acknowledgment: I have read and understand this agreement, am giving up certain legal rights including the right to sue, and agree voluntarily.
