I will be prioritizing flash designs, as well as any project that I can freehand. I am also open to custom designs, especially if they are a custom version of one of my fine China animals! I will be in Rigby tattooing September 9, 10th and 11th. I will be working 10AM - 5PM, each one of those days. Please make sure that you would be available during those dates and times, if you have a specific day and time that you were wanting to come in, please request that in the box down below. If you are free anytime, I would also love for you to list that so I can keep that in mind when it comes to trying to fit as many of you guys in my schedule as possible!

Do you have any medical issues that I need to be made aware of that could possibly interfere with a tattoo such as a blood thinning disorder, blood clotting disorder, HIV or AIDS, currently on Accutane or have taken a dose of Accutane in the last six months? It is also recommended to wait at least 4 to 6 weeks after being under anesthesia to get a tattoo, and 4 to 6 weeks to get a tattoo after being on antibiotics. Please keep that in mind when it comes to booking, and if you have any medical procedures, you think will interfere please list below.

Do you have any allergies that I need to be aware of?

In this section, if you are planning on getting tattooed in an area that is around other currently existing tattoos please attach a few photos of the area you were wanting to get tattooed. If you are getting a flash design, you do not need to worry about attaching a photo, unless you were worried about the design fitting around your other tattoos, then please attach a photo and I can chat with you about what’s going to fit in the area!
It’s best to have somebody else take the photos for you while you are standing up straight, please attach a photos from a few different angles. Arms and legs are pretty easy to get photos taken if you have assistance, if you are attaching a photo of your back, please do not have any bras or shirts on and just send a direct flat image of your back. (if you are planning on getting tattooed in an area that you feel like shouldn’t have a photo attached please don’t😅)

Also, this is a reminder that this is just a request form and I will be going through all my inquiries and selecting people for my schedule!
