✨ Who can Participate: This service is offered to caregivers with a new placement of a child(ren) in your household ages newborn to 18 with child welfare involvement and youth who have aged out of foster care up to age 21 with proof of previous or current child welfare involvement.

REQUIRED PROOF:

Attach any of the following documents as proof of current child welfare involvement:

Notice to Provider (NTP)
Tribal Child Welfare Involment from any of the 22 tribes
Aged Out Youth: Proof of previous or current child welfare involvement
