In participation of player tryouts organized by Wesley Chapel Storm Baseball, I, the undersigned parent or guardian of the listed minor, acknowledge and agree to the following:

Assumption of Risk: I understand that participation in tryouts involves risks including physical injury. I voluntarily and knowingly assume all such risks associated with my childs participation.

Medical Treatment Authorization: I authorize Wesley Chapel Storm Baseball and its representatives to seek and obtain any necessary medical treatment or attention in the event of an injury, illness, or medical emergency during the tryout. I understand that league representitiaves will make reasonable efforts to contact me  before seeking medical treatment, but they may proceed with necessary medical care if immediate attention is required.

Photography and Publicity Release: I grant Wesley Chapel Storm the right to use photographs, video or any compilation of the previously listed, of myself and/or my child during tryouts, for use on their Social Media Platforms. 

Parent/Guardian Consent: Since the the above listed participant is a minor, a parent or legal guardian must consent to this waiver and release of liability of Wesley Chapel Baseball on their behalf.

I have read and understand this Waiver and Release of Liability, and by signing this document, I agree to the above terms. I am aware that by signing this document, I am waiving certain legal rights that I or my heirs, executors, administrators, and assigns may have against Wesley Chapel Storm Baseball.
