Program Description
I understand that my child is participating in The Luck Group’s Youth Basic Lawncare & Maintenance Workshop, a hands-on educational program designed to teach youth basic lawncare skills, outdoor safety, tool awareness, responsibility, teamwork, entrepreneurship, and practical life skills.

Activities may include, but are not limited to:

• Safety orientation and instruction
• Basic lawncare and maintenance demonstrations
• Hands-on instruction using or observing outdoor equipment
• Push mower safety and operation
• Riding mower safety demonstration and/or supervised instruction
• Weed eater/string trimmer safety and operation
• Yard cleanup, debris awareness, and outdoor worksite safety
• Entrepreneurship and financial education
• Group discussion, reflection, lunch, and certificate presentation
• 2.Acknowledgment of Risks
  I understand that participation in outdoor lawncare and maintenance activities involves certain risks, even when safety rules are followed. These risks may include, but are not limited to:
• Cuts, scrapes, bruises, blisters, strains, sprains, or falls
• Contact with grass, weeds, dirt, dust, insects, plants, pollen, or outdoor allergens
• Heat, sun exposure, dehydration, fatigue, or weather-related conditions
• Noise, vibration, or flying debris from lawncare equipment
• Injury from improper use of tools or equipment
• Slips, trips, uneven ground, holes, rocks, sticks, or other hazards
• Contact with other participants, instructors, volunteers, or equipment
• Transportation-related risks if transportation is provided or arranged separately
• I understand that The Luck Group will provide safety instruction, supervision, and reasonable precautions, but no activity can be guaranteed to be completely risk-free.

3. Parent/Guardian Consent to Participate
I certify that I am the parent or legal guardian of the minor participant listed above and that I have the authority to give permission for this child to participate.

I give permission for my child to participate in the Youth Basic Lawncare & Maintenance Workshop and related activities conducted by The Luck Group, its staff, instructors, volunteers, partners, sponsors, and approved representatives.

4. Participant Health Statement
To the best of my knowledge, my child is physically able to participate in this workshop. I agree to notify The Luck Group in writing of any allergies, medical conditions, physical limitations, medications, behavioral concerns, or special accommodations that may affect my child’s participation.

5. Safety Rules and Behavior Expectations
I understand that my child must follow all safety rules and instructions given by The Luck Group staff, instructors, volunteers, and workshop partners.

I understand that my child may be removed from an activity or from the workshop if they:

• Refuse to follow safety instructions
• Use tools or equipment in an unsafe manner
• Engage in horseplay, fighting, bullying, or disruptive conduct
• Leave the supervised area without permission
• Create a safety risk to themselves or others
• 
  6. Emergency Medical Authorization
  In the event of an emergency, I authorize The Luck Group staff, instructors, volunteers, or representatives to seek emergency medical treatment for my child if I cannot be reached in time. I understand that every reasonable effort will be made to contact me or the emergency contact listed above.

I understand that I am responsible for any medical expenses not covered by insurance.

7. Assumption of Risk and Release
I understand the nature of the workshop and the risks described above. On behalf of myself and my minor child, I knowingly and voluntarily assume the risks associated with my child’s participation in this workshop.

To the fullest extent allowed by law, I agree not to hold The Luck Group, its directors, officers, employees, instructors, volunteers, partners, sponsors, property owners, or representatives responsible for ordinary risks associated with participation in the workshop, except where prohibited by law.

I understand that this form does not waive rights that cannot legally be waived under applicable law.

8. Transportation

9. Lunch, Food, and Allergy Notice

10. Parent/Guardian Certification
By signing below, I confirm that I have read and understand this form. I voluntarily give permission for my child to participate in The Luck Group’s Youth Basic Lawncare & Maintenance Workshop.

Photo, Video, and Media Release

I understand these images may be used for nonprofit purposes, including social media, newsletters, grant reports, flyers, promotional materials, donor updates, and the organization’s website.

I, the undersigned participant, affirm that I am of the age of 18 years or older, and that I am freely signing this agreement for myself and all participants mentioned in the above registration form. I certify that I have read this agreement, that I fully understand its consent and that this release cannot be modified orally. I am aware that is a release of liability and a contract and that I am signing it of my own free will.
