Please complete this form and acknowledge the liability waiver and your consent to participate in wellness activities in the park. You will have the option to enter a raffle (must be present to win) and make a donation online. Cathedral Square Wellness Weekends Participant Registration, Waiver, Release of Liability & Raffle Entry by participating in Cathedral Square Wellness Weekends (“Event”), I acknowledge and agree as follows: My participation in the Event and related wellness, fitness, yoga, exercise, and recreational activities is completely voluntary. I understand that participation in physical activities involves inherent risks, including but not limited to muscle soreness, strains, sprains, falls, physical injury, illness, permanent disability, and death. I knowingly and voluntarily assume all risks associated with participation.To the fullest extent permitted by law, I hereby release, waive, discharge, and hold harmless East Town Association, Milwaukee County, Cathedral Square Park, event organizers, instructors, volunteers, sponsors, vendors, and their respective officers, employees, agents, representatives, and affiliates from and against any and all claims, liabilities, damages, losses, or causes of action arising out of or related to my participation in the Event, including claims arising from ordinary negligence. I certify that I am physically capable of participating in these activities and understand that I should consult a physician before engaging in any fitness or wellness program if I have any medical concerns.
