• PCR Member Satisfaction Survey

    Fall 2025-Spring 2026

  • PCR is committed to providing high-quality, person-centered programs that are designed to meet our members' needs and preferences. This survey will help PCR fulfill that commitment to you. PCR wants and needs to hear from our members and their caregivers, so please take a moment to participate in this survey.

  • Your participation is especially important in light of the fact that PCR must submit member satisfaction survey data results to the Montgomery County Department of Health and Human Services. All data will be aggregated and reported anonymously. Please reply by Friday, June 19th. Thank you very much.

For members participating in PCR programs via Zoom, how much has participation in PCR virtual programs increased you or your family member's level of independence; for example, being able to log in and participate in Zoom programs with less support, or gathering needed materials in preparation for the Zoom program?
