Stay Rooted Repair Fund Application
The Stay Rooted Repair Fund is a no-cost roof replacement program for eligible owner-occupied households in Milwaukee's Lindsay Heights neighborhood. The program is funded by the Zilber Family Foundation and administered by One 5 Olive.

Please have the following documents ready to upload before starting your application.
- Government-issued photo ID (driver's license, state ID, or passport)
- Proof of occupancy (utility bill or similar document showing your name and property address)
- Income verification (pay stub, bank statement, or benefit statement dated within the past 90 days)
