Assumption of Risk and Release of Liability
I, the undersigned participant (and parent/guardian if participant is a minor), acknowledge that participation in baseball instruction and training activities with The Pitching Lab LLC involves inherent risks of physical injury, including but not limited to, extremity injuries, collisions, falls, impact with equipment, and other participants. I voluntarily assume all risks associated with such activities and agree to release, indemnify, and hold harmless The Pitching Lab LLC, its coaches, staff, and affiliates from any and all liability, claims, or demands arising from participation, whether caused by negligence or otherwise.
Medical Authorization
I authorize The Pitching Lab LLC staff to seek emergency medical treatment for the participant in the event of injury or illness. I understand that I am responsible for all medical expenses incurred.
