This form takes less than 10 minutes. Here's what we'll ask and what we'll do with it:

What we'll ask you:

- The name of your transplant center
- Who you would like to designate as your Champion — a trusted person in your life who will help invite your personal network
- The names and contact information of people in your life you'd like us to invite to the next Kidney Convo
What happens the moment you submit:
- We send your Champion everything they need to invite your personal network to a Kidney Convo
- We reach out directly to the contacts you submit on your behalf — so you don't have to
- Interested attendees attend A Kidney Convo-- a free, 90-minute education event and are connected to the donor intake process if interested.
You don't have to follow up. You don't have to make any calls. You just fill this out — and we take it from there.

A Champion is a trusted person in your life — a spouse, sibling, adult child, or close friend — who advocates on your behalf. Their job isn't to ask anyone to donate. Reaching out to faith-based communities

If you share your faith community's information with us, we'll reach out directly to a contact there and ask them to share an invitation for A Kidney Convo with their congregation. We'll handle the entire conversation — you don't need to say a word to anyone. Please know that while we will always make the ask, we cannot guarantee that every faith community will choose to share this with their members. Filling out this section does not guarantee your faith community will participate. But for those that do, their community will hear about living kidney donation in a supportive, no-pressure setting, and anyone who feels called to learn more can take the next step on their own.
