Authorization
By submitting this form the parent/guardian agrees to hold harmless The Mission Church, its employees and agents, for any injury to the minor(s) to damage to his/her or any property which may be incurred by or as a result of said participation. The parent or guardian affirms that the above name minor(s) has no health problems which would preclude his/her participation in said activity. In addition, for the unforeseen possibility that my child(ren) would get injured or sick and need medical attention, I request that I first be contacted, and if cannot be reached that medical attention is given to my child. Prompt attention to my child(ren) injuries or sickness is greatly appreciated.
