PARTICIPANT CONSENT FORM
Data Collection:
We will collect your personal information and answers with questionnaires and audiovisual recordings for future reference. All information collected in the study will be coded, stored, and processed anonymously. The data will be stored on a cloud server managed by the Keio University Graduate School of Media Design (KMD) - Future Crafts Project (PI: Prof. Junichi Yamaoka) until March 1st, 2030, after which it will be disposed of responsibly.
Risks and Benefits:
We guarantee no data misuse, and privacy is completely preserved.
Participants' Rights:
You have the right to omit or refuse to answer or respond to any questions during an interview, discussion or questionnaire form. You have the right to pause or terminate your participation at any time. You have the right to have your questions regarding the procedure answered. If you have any questions as a result of reading this information sheet, feel free to ask the researcher before the interview, discussion, or workshop begins.
Confidentiality/Anonymity:
If you have read this form and have decided to provide your consent, please understand that the results of this study may be presented at scientific or professional meetings or published in scientific journals. Your identity will not be disclosed unless we directly inform you and ask for your permission.
